Choose one component of the Framework for Safe, Reliable, and Effective Care.

One component of the Framework for Safe, Reliable, and Effective Care is Patient and Family Engagement. This component refers to the active involvement of patients and their families in the healthcare process. This can include providing patients with information about their condition and treatment options, involving them in decision-making, and gathering feedback on their care.

Describe the component

Patient and family engagement is important because it can lead to better health outcomes, improved patient satisfaction, and more efficient use of healthcare resources. When patients and their families are engaged, they are more likely to understand their condition and treatment, follow through with recommended care, and be satisfied. Additionally, involving patients and their families in the healthcare process can help to identify and address any barriers to care, such as lack of transportation or difficulty understanding medical information.

There are many ways to achieve patient and family engagement, including involving them in care planning, providing clear and understandable information, and creating a welcoming and respectful environment. It also includes involving patients and families in designing, implementing, and evaluating care.

What is the nursing leader’s role in implementing the component?

The nursing leader plays a vital role in implementing the Patient and Family Engagement component. They are responsible for creating a culture that values and supports patient and family engagement and providing the resources and support necessary for nurses and other healthcare providers to effectively engage patients and their families in the care process.

Some specific responsibilities of the nursing leader in implementing Patient and Family Engagement include:

  • Setting the example: The nursing leader should lead by example, engaging patients and their families in their care and creating a culture that prioritizes engagement.
  • Providing education and training: The nursing leader should ensure that nurses and other healthcare providers have the knowledge and skills to effectively engage patients and their families in the care process.
  • Encouraging communication: The nursing leader should promote open and honest communication between patients, families, and healthcare providers and create an environment where patients and families feel comfortable sharing their concerns and feedback.
  • Identifying and addressing barriers: The nursing leader should work to identify and address any barriers to patient and family engagement, such as lack of transportation or difficulty understanding medical information.
  • Creating a welcoming and respectful environment: The nursing leader should create a welcoming and respectful environment that promotes patient and family engagement, such as providing a comfortable and private space for patients and families to meet with healthcare providers and providing translated materials for non-English speakers.
  • Involving patients and families in the care planning process: The nursing leader should include patients and families in the care planning process and encourage their participation in the design, implementation, and evaluation of care.
  • Measuring and evaluating the effectiveness: The nursing leader should measure and evaluate the effectiveness of patient and family engagement efforts and make adjustments as necessary.

Apply it to a clinical situation that you have experienced.

A clinical situation I have experienced where the nursing leader’s role in implementing the Patient and Family Engagement component can be applied would be when a patient is admitted to the hospital for surgery. The nursing leader is responsible for the patient’s care during their hospital stay. In this situation, the nursing leader’s role in implementing Patient and Family Engagement could include:

  • Setting the example: The nursing leader should lead by example, engaging the patient and their family in the care planning and creating a culture that prioritizes engagement.
  • Providing education and training: The nursing leader should ensure that the nursing staff has the knowledge and skills to effectively engage the patient and their family in the care process, such as providing clear and understandable information about the patient’s condition and treatment options.
  • Encouraging communication: The nursing leader should promote open and honest communication between the patient, their family, and the healthcare team and create an environment where the patient and their family feel comfortable sharing their concerns and feedback.
  • Identifying and addressing barriers: The nursing leader should work to identify and address any barriers to patient and family engagement, such as language barriers or difficulty understanding medical information.
  • Creating a welcoming and respectful environment: The nursing leader should create a welcoming and respectful environment that promotes patient and family engagement, such as providing a comfortable and private space for the patient and their family to meet with healthcare providers and providing translated materials for non-English speakers.
  • Involving patients and families in the care planning process: The nursing leader should involve the patient and their family in the care planning process and encourage their participation in the design, implementation, and evaluation of care.
  • Measuring and evaluating the effectiveness: The nursing leader should measure and evaluate the effectiveness of patient and family engagement efforts by gathering feedback from the patient and their family and making adjustments as necessary.

The nursing leader plays a vital role in ensuring that the patient and their family are engaged in the care process and decision-making. This can lead to better health outcomes, improved patient satisfaction, and more efficient use of healthcare resources.

How well does your organization align with the framework?

HCA Healthcare has been named one of the World’s Most Ethical Companies by Ethisphere 12 times since 2010. (Hca Healthcare Ethics and Compliance, 2023) HCA has been awarded many times for its ethics and compliance program. They ensure that their actions always reflect their mission and a fundamental commitment to their stakeholders and the code of conduct. HCA is very proud of its ethics and compliance program. I feel that they are well aligned with the framework of the IHI. (Hca Healthcare Ethics and Compliance, 2023)

In which areas does the organization perform well?

The company implements very stringent safety standards as well as provides education and continuous education free for employees as well as per diem nurses. HCA constantly conducts studies and evidence-based practice guidelines to keep patients and staff safe.

What areas need improvement?

The areas that need improvement are the patient-to-nurse ratios, the retention of nurses and nurse leaders, and upper management. I have found that because I travel between HCA facilities working for Healthtrust, there is a constant turnover rate. I worked at a hospital, and within one year, the CNO changed 3 times; there were always new nurses and new nursing leaders. I think if HCA paid their employees better, the retention would be higher. I was born twice what a staff nurse makes and work for the same company.

Explain how nursing leadership is involved in these areas.

I have encountered many different types of nurse leaders, managers, and directors since I have been a per diem for HCA. I noticed that the turnover has increased since COVID-19, and burnout has increased. Predicting nurse turnover is important to prevent expensive and avoidable staff loss.

One factor that may influence nurse turnover is nurse managers’ leadership styles. Three main leadership styles have been identified: transactional, in which leaders give contingent rewards; transformational, in which leaders inspire and motivate; and passive-avoidant, in which leaders are absent.(Suliman et al., 2020)
