The major needs of the Chinle population include:

i.        The majority of the residents live in rural and remote regions away from major healthcare and educational facilities. Some of these regions are too remote with no educational or healthcare facilities to provide diabetes and obesity care and health information.

ii.        Most of the Chinle community and a majority of the Navajos live below the state and federal poverty line. An estimated 1,569 people, or 41.8% of the population of Chinle, live below the poverty line (Census Reporter, 2022).

iii.        Chinle residents living in remote and rural regions have limited access to reliable transportation. Some regions are beyond the main transit lines and have no accessible roads except for a few dusty roads.

iv.        A majority of the Chinle population has limited access to quality health information on obesity and diabetes.

v.        The Chinle community have significant distrust for Western medical practices due to the influence of the Dine medicine men. The Navajo beliefs about disease and illness, including the causes of such differ from Western medicine. Therefore, treatment strategies may conflict.

vi.        The Chinle population is affected due to cultural practices, poverty, and a lack of healthy dietary practices. A majority of the residents, due to economic disadvantages, noting that 41.8% live under the poverty line, are reliant on processed foods from fast food outlets.

vii.        The population has a high prevalence of chronic illnesses and conditions, including obesity, diabetes, and hypertension. The average BMI among the Chinle population is 29.1. 38.3% are obese, 12.7% are diabetic, 41.5% are at a risk of diabetes, and only 1.6% are taking insulin (city-data.com, n.d.).

The current high prevalence of obesity and diabetes among the residents of Chinle and the Navajos can be attributed to different socioeconomic factors. In Chinle, due to culture, most residents adopt a more holistic and traditional approach to healing rather than Western medicine.

Such a strong belief in traditional medicine means has contributed to most residents being reluctant to utilize or engage with modern medicines, leading to a higher risk of disease. At the same time, despite the advancement and potential of modern medicines to manage and treat the majority of conditions affecting the people of Chinle, the integration of such Western methods of care with traditional Navajo medicine must align with the cultural norms and values to gain wider social acceptance.

The process innovation aims to provide regular and fulltime support for health clinics in remote regions, support patients, and enable a collaborative team approach in diabetes and obesity management, as well as with other conditions. The process innovation will also support traditional Navajo health practices and Navajo medicine men in the delivery of traditional care combined with modern medicine.

Scenario
You are the director of the Navajo Area Indian Health Service (NAIHS), and you are responsible for improving the delivery of care for obesity in the Navajo Nation town of Chinle, Arizona.
