One health issue of concern in Mississippi is obesity, with the rate being 41 % among adults as opposed to 32% nationally (CDC, 2022). This high prevalence increases the incidence of chronic diseases such as hypertension, diabetes and heart disease, especially among people experiencing poverty and the societies underprivileged.

(B) Background

Studies show that Mississippians have poor dietary habits, lack of exercise, and high incidences of poverty as potential causes of obesity. The CDC’s High Obesity Program (HOP) is an evidence-based effort aimed at partnering with local entities to ensure effective community interventions that comprise nutrition interventions, promotional changes in physical activity, and enhancement of knowledge on health. (A) Assessment

The HOP strategy can effectively lower obesity in Mississippi by promoting healthy eating and physical activity and improving health literacy. By tackling obesity from its source, the program decreases the chance of developing chronic diseases, decreases expenditure on health, and enhances the well-being of people in Mississippi.

Introduction

Advanced professional nurses have the potential to improve health outcomes in patients and populations through interprofessional collaboration and healthcare advocacy. In this performance assessment, you will demonstrate competence as an advanced professional nurse to identify published data that supports a health issue, concern, or behavior in a city, county, or state of your choosing.

You will explore the role of the advanced professional nurse in developing and leading an interprofessional team to identify evidence-based policies, programs, or initiatives to improve the identified health issue, concern, or behavior in the chosen city, county, or state. You will identify a public leader with the authority to move a policy, program, or initiative forward. You will then create a summary of the recommendation using the following format: identification, summary, background, assessment, and recommendation (ISBAR) that you might propose to the public leader.

Requirements

Your submission must be your original work.
