The health information system (HIS), developed for healthcare data management, was one fundamental approach. For example, we replaced the paper version of the patient’s medical history with an Electronic Medical Record (EMR). It has been used to store patient information, test results, medication, and treatment options accessed through computer software or mHealth apps on smartphones and tablets. Nonetheless, my organization has used it to share patient information between departments and facilities, such as during referrals, to ensure patients receive the best solutions to their problems.

Furthermore, the acute care clinic implemented AlayaCare’s Remote Patient Monitoring (RPM) Solution, which provides our clinical staff with an instant dashboard to remotely monitor patients, reducing unnecessary visits and addressing patient needs. I recall a high number of readmissions at first, which significantly impacted our healthcare providers and the organization as a whole.
