We will begin by proposing an evidence-based care plan that can help to enhance safety and improve outcomes for Caitlynn. I will then explain the evidence-based model used to develop the care plan. After that, I will identify the most relevant evidence in developing the care plan. Finally, I will explain the benefits of remote collaboration and the strategies to avert the interdisciplinary collaboration challenges.

Evidence-Based Care Plan to Improve Safety and Outcomes

Let me take you through an evidence-based care plan to improve safety and outcomes. Healthcare providers and clinicians at Vila Health should utilize various strategies to ensure that patients receive adequate care at the hospital and remotely from home. The need for adequate nutritional therapy should be emphasized. Notably, according to Martin (2018), patients with cystic fibrosis usually have inadequate nutrition caused by persistent malabsorption, high caloric needs, or anorexia. Martin (2018) notes that this is demonstrated by loss of weight and a negative deviation from the ideal body weight by approximately 20%. In this context, Caitlynn’s weight is lower than the ideal body weight. According to Kapnadak et al. (2020), Caitlynn’s parents should modify her dietary intake to include adequate hydration, liberal salt intake, a high protein diet, a high caloric diet, and fat-soluble vitamins such as A, D, E, and K. essentially, Kapnadak et al. (2020) note that adopting this nutritional approach helps to address the increased dietary requirements for the patient and malabsorption that accompany cystic fibrosis.

Nurses should collaborate with respiratory therapists to administer oxygen therapy to address impaired gas exchange. The parents should be advised on the importance and technique of placing the child in a semi-Fowler’s position (Kapnadak et al., 2020). This position enables adequate expansion of the lungs and prevents the collapse of airways and shortness of breath (Martin, 2018). Parents should ensure that they rest between routine tasks to decrease oxygen needs. Furthermore, the parents should be familiar with the pulse oximeter and interpretation of its readings (Martin, 2018). This will help to monitor oxygen saturation levels. Ineffective clearance of the airways is caused by mucus plugging facilitated by the rapid growth of bacteria in the respiratory system (Jessup et al., 2018). This can manifest as crackles, difficulty in breathing, and lowered pulmonary function tests (Martin, 2018). The respiratory therapist should utilize various airway clearance therapies to manage the patients. Examples include chest physiotherapy, postural drainage, and the utility of chest percussions (Martin, 2018). The parents should be taught chest percussions and postural drainage techniques to be used by the child Kapnadak et al. (2020).

Kapnadak et al. (2020) reported that mucolytics such as dornase alfa and bronchodilators such as albuterol are important. Healthcare providers should use these medications to manage patients with CF. Inhalation techniques using devices such as metered dose inhalers will allow adequate delivery of medication o the airways (Jessup et al., 2018). Pharmacists should educate parents on the inhalation techniques that maximize drug delivery for the child (Jessup et al., 2018). Healthcare providers should indicate pancreatic enzyme replacement therapy and anti-pseudomonal antibiotics such as fluoroquinolones (Kapnadak et al., 2020).

EBP Model to Help Develop Plan of Care

The evidence-based model used to develop this care plan is the Stetler Model. The first step of this model involves preparing and identifying the problem and the role of the evidence-based practice. (Camargo et al., 2018) In this context, the problem is cystic fibrosis, and evidence-based care aims at patient outcomes in the hospital and away from the hospital. The second step entails searching and evaluating the credibility of the sources (Camargo et al., 2018). In this context, I used the CRAAP criteria to validate the sources (Esparrago-Kalidas, 2021). The third step involves decision-making. In this context, I compared information from different journals and identified the best evidence-based practices. The next step entails the application of the findings. During this step, I identified how evidence-based practices can be adopted by healthcare providers and parents of the child. The final step involves evaluation (Camargo et al., 2018). I utilized this evaluation phase to identify the goals of the care plan, which is to improve the patient’s outcomes and quality of life through appropriate management of CF. The evidence-based practices help fulfill the care plan’s goals, hence relevant in this scenario.

Most Relevant and Useful Evidence

The most relevant evidence is one that addresses impaired gas exchange and ineffective airway clearance. Impaired gas exchange and impaired airway clearance are a concern for patients with CF because it inhibits tissue perfusion and can lead to unconsciousness, coma, and death if it is not addressed promptly (Kapnadak et al., 2020). It is characterized by cyanosis, pallor, and difficulty in breathing (Kapnadak et al., 2020). Therefore, interventions such as oxygen therapy at the hospital ensure sufficient oxygenation occurs. Using a pulse oximeter to monitor oxygen saturation ensures that necessary interventions that prevent inadequate oxygenation are initiated (Jessup et al., 2018). Placing a patient in a semi-Fowler position promotes lung expansion and ensures adequate ventilation (Jessup et al., 2018). Airway clearance therapy facilitates the removal of the tenacious, thick mucous commonly found in patients with cystic fibrosis (Kapnadak et al., 2020). These interventions help to improve tissue perfusion and promote the well-being of the patients.

Benefits and Strategies to Mitigate the Challenges of Interdisciplinary Collaboration

Remote collaboration is advantageous to the patient and healthcare providers. In this scenario, the patient suffers from cystic fibrosis, a condition that requires regular visits to the hospital and an interdisciplinary approach to management. However, the patient lives far from the hospital. Remote collaboration enables real-time patient management by the members of the interdisciplinary committee. This flexibility reduces the possibility of health deterioration resulting from long trips to the hospital to address an emergency that could be adequately managed from home. It also enables interdisciplinary team members to monitor the patient perpetually and make relevant interventions that promote the patient’s well-being.

In this scenario, challenges in interprofessional collaboration are demonstrated by the lack of open communication and common telehealth technology among healthcare providers. This can be addressed by adopting collaborative tools such as Zoom or Doxy. me (Folkman et al., 2019). According to Rutledge et al. (2021), Doxy. me can permit an exchange of information among many healthcare providers. Subsequently, this eliminates the need for at least 100 persons to use telemedicine, as reported in this scenario. Healthcare providers should uphold open communication that ensures timely delivery and feedback concerning a particular problem. Ultimately, this helps to avoid delays in managing patients.
