Proposed Practice Question
The focus practice question for my literature review is: How do higher nurse-to-patient ratios than current staffing practices impact patient safety outcomes of falls, pressure ulcers, and medication errors in adult hospitalized patients in acute care units at St. Michael’s Medical Center over six months? This question was chosen because of the ongoing problems with staffing shortages and their perceived impact on clinical safety and the quality of care within the organization (Twigg et al., 2021):Peer Feedback: Practice Questions and Search Strategies.
PICOT Format
In a PICOT format, the population (P) is adult patients in acute care facilities. The intervention (I) is the introduction of enhanced nurse-to-patient staffing ratios. The comparison (C) is existing nurse staffing levels.
The outcome (O) is decreased safety events of falls, pressure ulcers, and medication errors. The time (T) in which the outcomes will be assessed is six months. This framing facilitates evidence-based inquiry with quantifiable, time-limited outcomes.
Search Strategies
I carried out an extensive literature search of the Walden Library databases, including PubMed, CINAHL, and Embase. The keywords used were “nurse-patient ratio,” “nurse staffing,” “acute care,” “missed care,” “patient falls,” and “medication errors.” Boolean operators were used to expand and narrow the results (Alharbi & Stevenson, 2020). Filters were used to include full-text, English-language, peer-reviewed research published from 2020 to 2025.
Inclusion and Exclusion Criteria
The inclusion criteria were peer-reviewed research published between 2020 and 2025 that assessed nurse staffing and patient outcomes among adult patients in acute care. Exclusion criteria were pediatric or psychiatric patient populations, original research devoid of articles, or non-English articles. Quantitative and qualitative studies, as well as relevant non-research evidence, were included to build the synthesis.
Research and Non-Research Evidence
Research and non-research evidence were considered. Research evidence included qualitative, quantitative, and mixed-method studies with quantifiable patient safety and staffing outcomes (Wasti et al., 2022). Expert consensus reports and clinical guidelines were some non-research evidence used to support best practices. Including both enhanced the review’s usefulness and applicability in acute care environments.
