Organizational Structures in Nursing

Organizational Structures in Nursing

The organizational structure in my healthcare facility typically includes a hierarchy of positions and departments that work together to provide patient care. Executive leadership is at the top of the organizational chart, usually including a CEO, CFO, and CMO (Shaikh et al., 2023). These individuals oversee the facility’s overall operations and make high-level decisions related to finance, strategic planning, and clinical care. Below the executive leadership are multiple layers of management, depending on the size and complexity of the facility. This includes a chief nursing officer (CNO), who oversees the nursing staff and reports to the CEO or CFO. The CNO is supported by nurse managers or unit directors who oversee specific patient care areas.

Other departments in my healthcare facility’s organizational structure include pharmacy, laboratory, radiology, physical therapy, and respiratory therapy. Each department has its manager or director who reports to the executive leadership or to the CNO. Interdisciplinary teams are also included in the organizational structure of my healthcare facility. These teams include physicians, nurses, social workers, and other healthcare professionals collaborating to provide comprehensive patient care.

Chain Of Command Used In the Organization

The chain of command in my healthcare organization typically follows a hierarchical structure. At the top of the hierarchy is the executive leadership team, including the CEO, CFO, and CMO. The executive leadership team oversees the facility’s overall operations and makes strategic decisions related to finance and clinical care (Wang et al., 2022). Reporting to the executive leadership team is the Chief Nursing Officer (CNO), who oversees the nursing staff and ensures the delivery of high-quality patient care. The CNO is supported by nurse managers or unit directors who oversee specific patient care areas. As a nurse manager, I report to the CNO. My responsibilities include managing the nursing staff, ensuring compliance with policies and regulations, and ensuring the delivery of high-quality patient care. In turn, I have direct reports, such as charge nurses or staff nurses, who work under my supervision and provide direct patient care. I supervise their work, provide guidance and support, and ensure that they follow organizational policies and procedures.

Leadership Structure Used In the Organization

Based on the organization chart, a hierarchical leadership structure is used, with a clear chain of command from the executive leadership team to the CNO and then to nurse managers or unit directors. This hierarchical structure can be useful for implementing change by providing a clear line of communication and accountability (FAAN & FAAN, 2022). Suppose nurses have an idea for changing a process. In that case, they can bring it to their unit manager or nurse manager, who can then evaluate the suggestion and, if appropriate, bring it to the attention of higher-level leadership. The organization uses a shared governance model where nurses have a more direct way of making suggestions for change. They can bring their ideas to a shared governance council or committee, including representation from all nursing staff levels, including directors. The council or committee can evaluate the suggestion, discuss it with stakeholders, and implement changes if appropriate.

Importance of This Information to My Success

Understanding the organizational structure and chain of command of my healthcare organization is critical for my success as a nurse manager. By clearly understanding the organization’s leadership structure, I can identify whom to report to, who reports to me, and whom I can turn to for guidance and support when needed. This understanding can help me navigate the complex hierarchy of the organization and ensure that I am in the proper channels when making decisions or seeking input from others.

Organizational Structures in Nursing

Step 1 Review your unit’s and/or facility’s organizational chart.
You have been promoted to nurse manager of your unit. You realize that to function effectively, you must have a better understanding of your organization’s structure and, specifically, the chain of command.

Find and analyze the organizational chart of the healthcare organization you are currently working for or for one that you have encountered in your nursing experience.

Step 2 In a two-page (500-word) paper, address the following points:

Give a brief description of the organization of the healthcare structure used in your facility.
Describe the chain of command used in your organization and identify the people you report to and those who report to you.
Consider the following and discuss how the chain of command is used to implement change.
Based on the organization chart, what leadership structure is used in the organization—hierarchical or flat? How does this affect implementing change?
Does this organization use a shared governance model for nursing?
When nurses have an idea for changing a process, and they make a decision as a group, where do they go with their suggestions? (the director? The unit manager?)
