Quality improvement is an ongoing project at my facility. One focus area in need of immediate improvement is medication errors and near misses, especially those directly related to transcription errors. Therefore, I have selected this problem for my practicum project. The quality improvement model I intend to apply to this problem is the FADE model. Spath (2018) describes FADE as an improvement model that focuses on an identified problem, solutions to correct the problem, and monitoring the outcomes of the implemented interventions.

The FADE model is comprised of four steps:

Focus: For this project, medication errors and near misses resulting from transcription errors is the problem selected. Through collaboration with nursing staff, senior leadership and executive leadership, better practices will be developed and implemented, accompanied by the provision of training and continued education for nursing staff to decrease the rate of errors.

Analyze: Generated reports outlining the number of reported medication errors and near misses and in-depth reviews of workflow processes will provide the data necessary for establishing factors that are contributors to this problem. Analyzing this data will render an opportunity to better understand the problem and gather data regarding the transcription process in question (Spath, 2018).

Develop: Through continued collaboration with key stakeholders, a solution to prevent medication errors and near misses by mitigating transcription errors will be created. An implementation plan will be developed once a solution is selected. Resistance will be inevitable, so it is crucial that staff training and education sessions be included in the implementation plan. According to Lundy & Morin (n.d.), resistance is often identified as a key reason for the failure of over two-thirds of change projects and is therefore considered an integral part of change management.

Execute: The goal of this intervention is solely centered on lowering the average number of medication errors related to transcription errors to 0%. It is in this phase that the intervention will be implemented, monitored, measured, and adjusted as needed.

My collaboration with the director of nursing and the quality control officer has provided me access to data that I previously thought was non-existent. The senior leadership is excited to assist me in successfully executing my intervention plan as they hope it will mitigate a serious problem that has not been addressed as expeditiously in the past. I feel that using this improvement tool as a guide will allow me to successfully meet the expectations of the stakeholders, improve the quality of care provided, and increase the level of patient safety.
