Harm is noted to be any physical or mental damage or injury (Merriam-Webster, n.d.). Keeping patients safe from harm is of the utmost importance to all nurses in practice. There are many ways in which nurses do this daily, such as checking medications before administration, listening to patients’ concerns, assisting patients with activities of daily living, turning ventilated patients, and providing education on disease processes and plans of care. However, one thing that can be done to reduce harm, which is not strictly followed, is the performance of a bedside shift report.

Bedside shift reports are performed not only to hold nurses accountable but also to decrease the risk of injury and improve patient outcomes. Nevertheless, nurses do not always follow this guideline for care during shift change. Roughly 80% of medical errors are attributed to miscommunication (Joint Commission on Accreditation of Healthcare Organizations [Joint Commission], 2012). Due to this statistic, an investigation of the effects on patient outcomes from ineffective communication is warranted. According to G. McEarl (2019), inconsistent bedside report with miscommunication leads to the common trends of inaccurate medication rates, mislabeled tubing, and, even worse, iv infiltrations. Therefore, a practice change should be initiated which mandates a uniform bedside shift report to prevent these events from occurring.

In order to test the aforementioned change in practice, the researcher could ask, “What effect does a mandated uniform bedside shift report have on the occurrence of iv infiltrations”? This type of research would be quasi-experimental as it not only helps determine a cause-and-effect relationship but also test the effectiveness of a nursing intervention to improve a patient outcome (Gray, Grove, & Sutherland, 2017). This approach would be better than one of experimental research as experimental research requires a highly controlled setting, and in this clinical setting, all variables cannot be controlled. Further, correlational research would not be chosen either, as it looks at the nature of the relationship and not the cause and effect. Finally, a descriptive approach would not be utilized as it is generally conducted when little is known about a particular situation.
