Choosing a group of study subjects is the first step in implementing a care plan for your child. An opiate-addicted young adult population seeking treatment for addiction is the target group in this case. Anxiety, sleep deprivation, and imbalanced nutrition are the nursing diagnoses that I have focused on. This was due to their vulnerability to death and disease associated with drug abuse. In addition, drug addiction impairs the immune system of an individual, making it difficult to fight opportunistic infections among patients. These topics were chosen because they are the ones that I am least passionate about. For me, it was a matter of personal growth and gaining a better understanding of what these patients are going through and feeling. Was it influenced by their sobriety? What are their future expectations? The most important thing was that I wanted to influence, educate, and encourage them over the course of these ten weeks.

Implementation of the RISE program is part of the care plan. A series of tests will be performed on substance-addicted patients to identify the underlying medical conditions as well as the extent of their substance use. Assessment of addiction and recovery will also be part of the diagnostic process. As a result, anxiety, sleep deprivation, and malnutrition will be identified, as well as their symptoms, causes, and severity, according to the report. Multiple health risks, including altered judgment and physical and brain functioning, could result in risky behaviors and dependency if not addressed. Self-medication and peer pressure are among the other health risks (Kalashnikov & Artamonov, 2020).

As a result of the implementation of the care plan, the patient will receive appropriate treatment for anxiety, sleep deprivation, and poor nutrition. A pharmacological and psychological approach is used to treat these diagnoses in order to achieve an effective management of the conditions as well as to treat the symptoms and opioid addiction among the participants. Also, patients will resume their participation in productive activities, attend one group meeting per week, submit to one drug test per week, and maintain their recovery by abstaining from drug abuse and engaging in risky behaviors. Both their psychosocial aspects and quality of life should also have improved during and after implementing the care plan. Selected interventions, participants, and resources will be identified, as well as a suitable site for implementation (Steingard, 2020).

The intervention’s key features will be identified so that they can be closely monitored and adhered to. An evaluation of effectiveness will be conducted after the implementation phase (wheeler, 2014). It will be necessary to collect information about patients. The diagnosis and treatment for symptoms and pain management, social support and coping, and assistance in transitioning to community-based care will be provided in the future. Implementation will require multiple resources. A medical diagnosis and treatment may require the services of a nurse or physician. For example, tests, medications, and psychiatric services must be purchased, and healthcare providers must be compensated. These treatments, tests, and medications are free of charge to the patients because the RISE program is publicly funded. The use of technical tools that help manage opiate addiction is also made possible through the program’s use of professional knowledge. Using these resources, the care plan will be implemented over the next four weeks, according to the plan. The patients will have enough time to adjust and adapt to the new changes, and the measurement of the results will be effective and efficient after this period. The RISE program itself and the treatments it offers have no expiration date, and neither does the RISE program itself. The RISE program allows patients to receive treatment as long as they remain sober. If a patient fails two drug tests in two weeks, he or she must sign a contract stating that they will not fail another drug test without being dismissed from the program.
