NR 505 Week 1 – Reflection on Learning
Week 1
Some experiences I have had with EBP in my practice areas include hand hygiene and oxygenation of the COPD patient. As a previous clinical manager on a med surge floor and ICU, we performed frequent hand hygiene audits on staff to make sure we complied, providing the best patient care and preventing the spread of infection. I can remember when I became a nurse, we were taught that the COPD patient should have oxygen between 2-3 l/m BNC to prevent hypercapnia. What I have learned is that it is true, but if the patient is not breathing well and O2 sats are below baseline, we can apply a nonrebreather in the long-term setting prior to sending to the ED. There have been many times that staff has come to me or been involved in a situation and has failed the patient’s needs for oxygenation because of lack of knowledge. Some factors in my practice area that support EBP would include policy and facility protocols based on best practices. One barrier with EBP that I see is when policies change, not everyone is aware of the changes, creating gaps in new knowledge. My role as an NP would be to stay current on EBP and research to be able to provide effective care to my patients.
Week 2
One challenge that I experienced with developing my PICOT question from my area of interest is that I chose an area that is very broad and has many entities. I had to look at my area of interest and narrow down a population I would use, decide if my prior interventions would be effective for this population, compare smoking and not smoking in this population, ensure my outcomes would be measurable, and if I would be able to use a timeframe for my population interventions. This assignment was very educational and allowed me to use critical thinking and increase my knowledge of my area of interest.
The PICOT question development could be applied in my current practice setting in the ED and as a future nurse practitioner in a clinic. The PICOT question allows me to recognize a practice problem, expand on current research, and educate patients on smoking cessation. Currently, I have direct care for my patients in the ED; during their visits and discharge, there are educational opportunities. This week’s assignments were overall interesting and improved my knowledge of PICOT.
Week 3
Personally, I felt like it was hard to find a quantitative article on my area of interest population but not on smoking cessation. I am still unsure if my article is correct. I have reviewed the NP Cafe module 3 information, and the example was helpful; it allowed me to review the example article and my article to see what I am looking for to complete the assignment. I was able to follow the Johns Hopkins Nursing EBP, Appendix E, Evidence Appraisal tool, but I was unsure if I was doing it right.
The article was appropriate for my study using a questionnaire regarding mobile smoking cessation apps. If I had help, I feel that the study could be replicated, but I would narrow down the respondents. This definitely would not be my expertise, but I could make it happen. The use of a mobile smoking cessation app would cover a lot of the younger and middle-aged population and would be useful with how people’s busy lifestyles are today. A large part of the population or generation today has a smartphone or access to one, making the mobile app easy to use.

Week 4
I feel that I have a better understanding but some uncertainty about what I am doing. This is a weak area for me. The qualitative study was easier to find than the quantitative study related to smoking cessation. Initially, I had difficulty until after reading the course work and examples in the Collaboration Café for this week’s assignment. While completing the assignment, I followed the example article, and it helped me locate information in my own article. The qualitative approach is appropriate for my area of interest. I would be able to replicate it in my practice area for smoking cessation. Overall, I feel that each research design would be helpful in my area of interest, but the best, I would say, is the qualitative study. This has been another great and educational week.
Week 5
For the assignment this week, it has been very difficult to locate ten articles on my PICOT population and in the US. I have not had any trouble finding evidence related to my area of interest, smoking cessation. Most of the articles I have located have been in Australia, Sweden, and so forth. Qualitative studies have been the easiest to find. I have found one article that was a mixed method. The overall quality of the evidence was good. I would like to see more cost analysis from smoking cessation and more support for the low socioeconomic populations since most evidence shows that this population has a higher dependence on smoking. I feel as if the literature supports a practice change.
Week 6
The CITI modules were brief and informative, and the quizzes were fairly easy to answer. It was interesting that it offered the actual presentation or reading information. I used both to complete the modules. I can’t really think of anything that surprised me. The CITI module held the researcher accountable for protecting the participants and treating them with respect. The ANA Code of Ethics holds nursing professionals accountable for their commitment to their patients. Ethical principles are similar between the CITI modules and the ANA Code of Ethics. As a future NP and a nurse, I can see the applications of the study in my nursing practice.
Week 7
This week, I was putting all the information for my evidence-based practice change into a PowerPoint presentation. There was a lot of reviewing past weeks’ assignments for information for this assignment. My presentation could benefit by sharing my results with clinics, hospital staff, other healthcare professionals, and community support groups that focus on smoking cessation. This would be additional steps toward educating the people. This could be another form of education for educators and patients who need information on smoking cessation. There were a lot of learning and educational opportunities in this week’s reading and assignment.

Week 8
There is no real plan at the present time to promote personal and professional growth in evidence-based practice. As I complete the program and in the practice setting, I will continue with my education, with seminars and readings to be on top of the current research. I do plan on spreading the importance of smoking cessation with my patients, family, and friends because I feel it is something that people need to know about. This week, we are presenting our evidence-based practice change to the class on Kaltura, which is out of my comfort zone. This is also the final week for the class and preview week for the next class.
