The global health concern that has impacted my community, Pasadena, is HIV, with the number of cases documented coming second countrywide, with approximately 11,323 cases as of 2017 (Targethiv.org, 2017). Coupled with the high rate of homelessness, the health concern the Pasadena community is struggling with has also exacerbated the rate of mental health and substance abuse cases (Cityofpasadena.net, 2020). Given the enormity of the geographic area, the health services are available, but they are spread out, making access a major issue, thus the gravity of the health matter.
In my nursing practice, the surveillance data that could be pulled from an EHR (electronic health record) is the HIV-prevalence survey data. The HIV-prevalence survey surveillance data is critical for my nursing practice, and it can be used to analyze and project future trends and possible interventions (Targethiv.org, 2017). The surveillance data is continuously stored in the electronic health record over some time (Cityofpasadena.net, 2020), and it provides a basis with which to estimate the magnitude of specific health issues, portray historical development, and come up with a hypothesis before evaluating and monitoring control measures and changes.
The information from a tweet about the health concern in the Pasadena community emphasizes the need for an intense case system/management navigation program formulated and is likely to engage the community infected or affected by the health issue, thus establishing the measures taken in the future to control the rate of spread.

Lastly, the rate of spread of HIV in the Pasadena community has significantly dropped compared to surveillance data from the last decade. The surveillance and management programs formulated over the years in the community have seen an increase in awareness, as well as improvement in resource allocation for the creation of further awareness and intervention.
