As a proficient nurse, I would use the system’s theory for patient-centered care. As I would do the initial physical assessment upon admission, and I would evaluate for any nonverbal reactions or cues in order to get a sense of the emotional state. Mr. R. has gone through significant changes, both physical and emotional, so the goal is to understand that these changes all factor into treating him as a whole person (Hood, 2014). When there is a case of body image displacement, nurses need to consider privacy rights and ask patients if they would like their family present as an environmental factor. After any type of procedure, I always asses for pain and try to make the patient as comfortable as possible.
Patient education starts upon admission and is an ongoing process that continues even at the discharge date. I unequivocally think the “teach back” method would be extremely beneficial for Mr. R. While under my care, a nurse is first and foremost the patient’s advocate and he must value that role and responsibility (Massachusetts, 2010). Teaching Mr. R about self-care while performing nursing care goes hand in hand. Care planning and treatment orders are vitals for a new ostomy. I tend to always encompass family with patient care for an efficient recovery phase. I would ask Mr. R if he would like any family member/primary caregiver present during appliance care. While performing appliance cares, I would make sure to go slow and explain in detail everything I am doing and then would have them perform the care while in my presence a few times before discharge. When discharge occurs, I would go over the process again in detail, give a hand-printed copy of instructions, and in order to make sure all their questions were answered. Again, the “teach back” method is extremely valuable to making sure Mr. R and/or family members know exactly how to care for Mr. R’s new appliance. Follow-up instructions to contact health care after discharge are also included in discharge information.

I sense that I am considered ‘Proficient’ on Benner’s Landmark of Continuum in my present position. I am by no means an expert, nor do I have a gargantuan amount of experience. However, I am pleased with my performance with overall changes. In the case of Mr. R., I would consider myself to be autonomous in regard to caring for him. As a proficient nurse, I can formulate quick-thinking and effective decisions regardless of milieu factors. They understand how to prioritize and advocate for patients (Chamberlain, 2016). I consider myself better off by using nursing assessment skills, perceptiveness competencies, and working independently to provide optimal patient care for Mr. R.
