NR 351 Week 5 Discussion – Leadership to Improve Quality in Diverse Situations

This week’s discussion brings me back to my unit, where each nurse demonstrates Leadership every day during the interaction, communication, participation, and decision-making to provide unconditional care to our patients. That reflects, of course, on the reputation of our unit and the hospital in general. According to Hood, “The whole person is the supra system for multiple interacting subsystems.” To maintain units, departments, and hospital, Leadership need to be in each aspect of that structure because it affects each person’s performance, motivation, and satisfaction.

I believe that the Nursing Manager is a leader who has unique leadership qualities and management skills to provide a smooth working environment and, at the same time, meet all hospital requirements and policies. We have a new Nursing Manager in our unit who used to work as a staff nurse before us. She is young, intelligent, and fascinated by nursing. I believe it is a tough job to become a manager to your former co-workers. Different levels and perceptions of each other could bring more tension and even jealousy in our working atmosphere, and I give her significant credit for developing trust and respect as a new leader in our unit. I often feel that nurses do not understand what responsibility, stress, and commitment the nursing manager is going through every day to improve the quality of patient care and organize our unit accordingly.

Conflicts and misunderstandings between Patient Care Technician (PCT) and Nurses reflect negatively on the relationship between staff and degenerate quality of care. Communicating with the team and having meetings are essential aspects of being a manager, and that is what I feel is necessary to improve our unit. When meeting face to face, we can have open conversations, and each team member, as a leader, can bring an idea to improve communication and work as a team. According to Hood,” Because they are open systems, people exchange matter, energy, and information with the environment” (Hood, 2014, p.128).

As an example, yesterday, I was a Charge Nurse on the floor, and this was for the first time in my nursing career that I was a witness of nursing discrimination and assault. This happened to one of my co-workers, a Chinese nurse, who is a great example of professionalism and quality care. She has never been reported or had any complaints. She came to me to do an exchange with patients because her patient didn’t like her and, with screams and anger, refused her care. When I tried to find out what happened, I was shocked to hear, “I do not want that nurse to touch me; she does not know what nursing care means, and she is disgusting; I want to write a complaint about her care.” As a nurse in charge, I spoke with the patient and the patient’s husband, I assured them that I would take care of them now and I would call patient representatives to file complaints if they needed.

This was only the first step; secondly, I called a nursing supervisor and explained the situation. The patient representative came back to me and asked if the patient was mentally okay. I spent another two hours in that room communicating with the patient and trying to advocate for my nurse. With my effort and patients, I calmed the patient down, and she started to feel sorry and told me that she didn’t like Chinese people. I was a witness of discrimination against Jewish people in Ukraine when I was a child, and since that time, I have forgotten it is possible. Communicating with this patient reminded me of that time. I explained to the patient about cultural diversity in our hospital and about humanity and respect for each other. I tried to bring different views and perspectives to the patient’s attention, and I felt she understood something. No complaints were filed. Communication and advocacy for clients and colleagues can be a heartbreaking process that involves in Nursing Leadership position.

“Facilitation in the advocacy process requires that the advocate take responsibility to ensure the client has all the necessary information to make informed decisions and to support clients in the decisions they make. Suggested that an effective way for the nurse to facilitate growth in self and others is through values clarification” (Hood, 2014, p. 469).
