Summary of Work Performed / Learning Opportunities

Section I.
Population Served: Discuss the population served during this month.
In the second month, I mainly worked with adults at Serenity Recovery Centers who were transitioning from inpatient care to outpatient care. Many individuals in this group faced chronic substance use and experienced trauma, problems with housing, and mental disorders such as generalized anxiety disorder, major depressive disorder, and bipolar disorder. Since many clients were court-directed or advised by others, their willingness to participate often depended on those recommendations.
In my experience, more of my clients were having a difficult time with grief, loss of family relationships, and feeling that everything was hopeless for them. Therefore, creating personalized support and showing much understanding and patience were essential (Yao & Kabir, 2023). Patients at different stages allowed me to see how organizational and personal factors shape therapy results.
During this month, I noticed how culture and income impact recovery and became more thoughtful about how I changed my approach. To sum up, this reinforced my desire for fairness and client-friendly care.

Describe the activities you performed during this month. What areas of strengths/needs did you focus on this month? You may use bullet points.
- Facilitated one-on-one counseling sessions using cognitive-behavioral techniques to help clients identify thought patterns and create actionable coping strategies for managing anxiety, cravings, and depressive symptoms.
- Co-led group therapy sessions, guiding discussions on personal responsibility, relapse prevention, and the development of healthy support systems while managing varied group dynamics.
- Participated in diagnostic intakes, gathering psychosocial histories and developing initial clinical impressions under supervision. I improved in summarizing and prioritizing presenting problems.
- Wrote daily and weekly clinical notes and progress summaries, ensuring that documentation met agency standards and accurately reflected session content and goals.
- Contributed to staff case consultations and team meetings by offering feedback on client care strategies and discussing appropriate therapeutic responses.
- Attended a professional development session on trauma-informed care, applying principles learned to clients with post-traumatic stress disorder and histories of childhood trauma.
- Worked actively on improving clinical confidence, maintaining ethical boundaries, and integrating supervision feedback into direct client care.
Section III. Professional Learning Goals

The main goal I had this month was to get better at bringing evidence-based strategies to both individuals and groups. I used cognitive behavioral therapy methods, including noticing and correcting thought patterns that harm the client (Nakao et al., 2021). As a supervisor guided me, I grew more certain in using these approaches and became more aware of what my clients needed. A further objective was to develop skills in guiding groups, resolving conflicts, and promoting honest discussion among participants.
I met this goal by leading sessions as a team and paying close attention to and discussing group interactions during supervision. I also focused on making my documentation more efficient and precise by consistently writing timely, clear, and clinically applicable progress notes (Demsash et al., 2023). With my supervisor, I explored how to make ethical decisions, the limits of working with clients, and informed consent.
Because of these goals, I became more confident and ready to handle more responsibilities. I am more confident in offering competent and ethical care to my patients now.

Monthly Logs and Activity Summary Reports are important documents that tracks your hours accrued each month and provides us with information of the learning you are engaged in at your placement sites. Attachments include an empty log and examples for completing your logs.
You are required to submit longs monthly and include a summary at the bottom of the log responding to the prompts on the page. Please do not upload any other documents where your logs go. You will receive a zero for documents in the wrong assignment space.
Log Completing – You must use the following system when completing your logs. Log hours use quarter sections.

Monthly Logs and Summary Activity Reports
15 minutes- .25. 30 minutes- .50 45 minutes – .75 and 1.0 hour – 1.0 Using this system will make it easy to total your hours and track your hours as well. For example, If you meet with some one for an hour and 45 minutes you will write it as 1.75. Logs are reviewed and any fraudulent acts will be addressed. All total hours need to have individual activities.
