Section I.
Population Served: Discuss the population served during this month.
Throughout the first month at Serenity Recovery Centers, I interacted most frequently with adults attempting recovery from substance abuse issues, some of whom had depression, anxiety, or post-traumatic stress disorder. Most clients struggled with problems such as homelessness, not having a job or income, unstable family ties, or time served in prison. As pointed out by Yadav and Gunturu (2024), the patients required treatment to be based on trauma and to observe strict therapeutic boundaries.
It was necessary to build rapport, show validation, and pursue a flexible pace to meet the needs of patients with various emotions and involvement in their care (Butt, 2021). I was required to be aware of different cultures, change my interventions according to the client’s progress, and stick with them during emotions or setbacks. Treating such a diverse group gave me a better understanding of addiction and mental health recovery. To conclude, I realized that having compassion, cultural skills, and being able to adapt is essential in any clinical practice.

Describe the activities you performed during this month. What areas of strengths/needs did you focus on this month? You may use bullet points.
- Conducted multiple individual counseling sessions focused on relapse prevention, emotional regulation, and self-esteem building. I practiced active listening, offered reflective feedback, and supported clients through various stages of change.
- Co-facilitated psychoeducational group sessions that explored coping skills, mindfulness practices, and managing interpersonal relationships in recovery settings.
- Participated in structured diagnostic interviews, helping gather client history, substance use patterns, and symptoms relevant to treatment planning.
- Completed detailed progress notes and clinical documentation immediately following sessions, ensuring accurate records of client interactions and treatment goals.
- Engaged in weekly individual and group supervision to reflect on clinical challenges, develop therapeutic strategies, and receive constructive feedback from my supervisor and peers.
- Collaborated in co-therapy sessions where I observed different counseling styles and learned to adapt my communication to various client presentations.
- Focused on improving foundational counseling skills, including rapport-building, summarization, and managing difficult emotional disclosures.
Section III. Professional Learning Goals
Identify the Professional Learning Goal(s) and discuss the activities completed to meet your professional learning goals.
This month, I worked toward several professional learning goals aimed at strengthening my core counseling skills and developing clinical confidence. I focused on learning better ways to examine patients and interview them by learning the DSM-5 and practicing with the help of supervisors. I discovered that using reflective listening, paraphrasing, and asking open-ended questions encourages clients to take part and supports their recovery (Braillon & Taiebi, 2020). Another primary aim was to identify the care requirements for clients who experience both addiction and other mental health problems.
I managed to reach this goal through joint sessions with colleagues, asking questions under supervision, and exploring evidence-based practices for integrated care. I worked on becoming self-aware, noting down my reactions, challenges from transference, and what I took from each session. As a result of these thoughts, I could direct my conversations with my supervisor and check how successful the therapy was. All in all, this month built a solid base for independent, knowledgeable, and client-focused clinical work.

Monthly Logs and Activity Summary Reports are important documents that tracks your hours accrued each month and provides us with information of the learning you are engaged in at your placement sites. Attachments include an empty log and examples for completing your logs.
You are required to submit longs monthly and include a summary at the bottom of the log responding to the prompts on the page. Please do not upload any other documents where your logs go. You will receive a zero for documents in the wrong assignment space.
Log Completing – You must use the following system when completing your logs. Log hours use quarter sections.

Monthly Logs and Summary Activity Reports
15 minutes- .25. 30 minutes- .50 45 minutes – .75 and 1.0 hour – 1.0 Using this system will make it easy to total your hours and track your hours as well. For example, If you meet with some one for an hour and 45 minutes you will write it as 1.75. Logs are reviewed and any fraudulent acts will be addressed. All total hours need to have individual activities.
