Intervention Presentation and Capstone Video Reflection
Hello, and welcome to today’s presentation. This presentation will highlight several elements of an intervention designed for a patient with Alzheimer’s disease (AD). Firstly, we will assess the intervention’s impact on the patient and her family. Secondly, we will explore research and the use of evidence. After that, we will discuss the application of healthcare technology. Also, we explore health policies that impact AD management. Fifthly, we will compare project outcomes to early assumptions. Finally, we shall evaluate professional and personal development.
Impact of the Intervention on Patient Satisfaction and Quality of Life
I had an interaction with my aunt. She is a 69-year-old African-American woman who was diagnosed with Alzheimer’s disease three years ago. She was accompanied by her spouse. Examination reveals reduced impulse control, as shown by the indiscriminate use of profane language, twitching muscles, and issues with perception-motor coordination. Her attention span has diminished in the last year. Furthermore, for the last six months, she has had difficulties recognizing close relatives. According to her spouse, the patient has been restless and wandering at night for the previous month. Also, her history shows she is not taking her medication as prescribed.
The intervention embraces pharmacotherapy and lifestyle modification. To begin, Galvin, Aisen, Langbaum, Rodriguez, Sabbagh, Stefanacci, Stern, and Vassey, writing in 2021, argued that pharmacotherapy provides adequate symptom control for mild to moderate Alzheimer’s disease. As such, early detection and treatment of Alzheimer’s disease can lead to a better prognosis. As De Marchi, Contaldi, Magistrelli, Cantello, Comi, and Mazzini reported in 2021, my intervention improved medication adherence by deploying telehealth measures such as audio messages and alarms. In this context, relevant pharmacotherapy includes donepezil and memantine. Lifestyle changes centered on nutrition and physical activity. Galvin, Aisen, Langbaum, Rodriguez, Sabbagh, Stefanacci, Stern, and Vassey also noted in 2021 that adequate nutritional standards include diets that contain fresh fruits and vegetables as well as moderate carbohydrates. Furthermore, adequate cardiorespiratory activity is relevant because it mitigates brain atrophy associated with AD, resulting in improved patient outcomes.
The patient and her spouse reported that they were content with the intervention. According to them, the intervention was unique because it would be their first interaction with telehealth technology. They were very excited about using mobile health and videoconferencing. They admitted that reminder messages and alarms would increase adherence to the treatment plan. They were also enthusiastic about the usage of videoconferencing to conduct real-time consulting sessions because it would increase access to healthcare. They reported that they would participate in at least four weekly sessions of moderate exercise. They stated that they were willing to follow the care plan because they felt that their autonomy was upheld.
The intervention improves the experiences of the patient and her family. In 2021, De Marchi, Contaldi, Magistrelli, Cantello, Comi, and Mazzini indicated that telehealth improves healthcare access by creating flexibility and allowing remote access to care. Healthcare access allows clinicians to make prompt interventions, hence, better patient outcomes. In this context, the patient and her family will leverage mobile health and videoconferencing. Furthermore, De Marchi, Contaldi, Magistrelli, Cantello, Comi, and Mazzini stated in 2021 that using mobile health features such as alarms enhances adherence to the treatment plan, resulting in optimal outcomes. Therefore, the intervention will improve the patient experience by allowing them to seek prompt healthcare, adhere to the care plan, and achieve better control of AD.
Evidence and Peer-Reviewed Literature
At this point, I will discuss how I used evidence and peer-reviewed literature to develop my capstone project. To begin, I gathered material from organizational websites published by government agencies, corporations, universities, and research institutions. In this context, I used websites such as CDC.gov and the Alzheimer’s Association. Also, online databases such as CINAHL and Google Scholar are available. The databases enabled me to personalize my searches to access peer-reviewed journal articles. I searched for relevant evidence using specified terms like “telehealth” and “health policy.” I used the CRAAP model to assess the authenticity of my evidence. Esparrago-Kalidas reports in 2021 that the CRAAP model focuses on the currency, relevance, accuracy, authority, and purpose of evidence. In this context, I selected the most current, accurate evidence that applies to my capstone project.
I used the principles of evidence-based practice to develop my capstone project. According to SCstatehouse.gov (n.d.), needs assessment is a principle of EBP. This principle allowed me to identify the issue of concern and its significance. In this scenario, ADs are the issue of concern. The other principles include intrinsic inspiration, intervention development, skill development, positive reinforcement, and performance feedback. Intrinsic motivation allowed me to interact with the patient and her family and urge them to uphold the intervention to improve AD management. In addition, intrinsic motivation allowed me to collaborate with the multidisciplinary team in formulating and implementing the intervention. Skills training allowed me to educate the patient and her family on the importance of adhering to the care plan and the appropriate use of telehealth technology. Also, it allowed me to educate the patient and her family about local community resources. I was able to determine and strengthen the drivers of adherence to the treatment plan using positive reinforcement. Lastly, performance feedback will allow me to evaluate patient outcomes and modify the intervention to achieve optimal results.
Health Care Technology
At this point, I will describe the application of healthcare technology to this capstone project. Authors De Marchi, Contaldi, Magistrelli, Cantello, Comi, and Mazzini, writing in 2021, reported that telehealth optimizes the management of AD via features such as mobile health, videoconferencing, and wearable devices. Mobile health is relevant to my capstone project because it improves information sharing and encourages adherence to treatment plans. This is accomplished via reminder messages, audio calls, and alarms. Authors De Marchi, Contaldi, Magistrelli, Cantello, Comi, and Mazzini writing in 2021 stated that wearable devices facilitate remote patient monitoring and allow clinicians to evaluate AD patients continuously. For example, wearable devices can be used to monitor patients’ physical activity, nutritional practices, and sleep-wake cycles. Furthermore, authors De Marchi, Contaldi, Magistrelli, Cantello, Comi, and Mazzini, writing in 2021, revealed that videoconferencing improves AD management by allowing patients to access care remotely. For instance, video conferencing can allow a patient to seek virtual real-time clarifications from their clinicians.
Several approaches can be used to optimize the use of healthcare technology in the future. Authors Vargas, Eguiguren, Mogollón-Pérez, Bertolotto, Samico, López, De Paepe, and Vázquez writing in 2020 report that training and education of key stakeholders such as clinicians and patients will improve the use of healthcare technology. Training and education increase digital literacy, hence better user experience. Furthermore, the availability of cost-effective, high-speed internet coverage will improve connectivity and allow clinicians and patients to use healthcare technology effectively. In addition, Vargas, Eguiguren, Mogollón-Pérez, Bertolotto, Samico, López, De Paepe, and Vázquez in 2020 reported that members of the public should be informed about the significance of healthcare technology in care delivery. Public education will create awareness and promote the effective use of healthcare technology.
Impact of Health Policy
At this point, I will talk about how health policies influenced my capstone project. To begin with, HHS.gov (n.d.) reveals that the National Alzheimer’s Project Act (NAPA) aims to minimize the cost of treatment while improving the care for AD patients. To achieve this, the NAPA champions more effective resource allocation to improve AD management. NAPA has six goals for the management of Alzheimer’s disease. HHS.gov (n.d.) reveals that the first goal is to improve Alzheimer’s disease prevention and management by 2025. The second objective is to enhance the effectiveness and caliber of care provided to AD patients. The third objective is to expand support programs for families and individuals with AD. The fourth objective is to raise public knowledge about Alzheimer’s disease. The fifth objective is to improve data monitoring for Alzheimer’s disease. The sixth objective is accelerating activities promoting healthy aging to reduce AD risk factors (HHS.gov, n.d.). My capstone project embraced NAPA by developing an individualized intervention. The patient and her spouse were involved in developing the treatment plan.
My capstone project also incorporated the Health Insurance Portability and Accountability Act (HIPAA). Authors Edemekong, Annamaraju, and Hayde, writing in 2022 revealed that HIPAA regulations apply to electronically transmitted personal health information (PHI). PHI includes patient identifiers such as medical history, diagnoses, medication, and laboratory testing. Authors Edemekong, Annamaraju, and Hayde writing in 2022, revealed that violating HIPAA’s rules will likely result in judicial procedures and penalties. My capstone project involves the transmission of PHI via telehealth technology. The privacy and security of patient data will be upheld via techniques like encryption and passwords. In addition, only authorized users will have access to PHI.
Nurse practitioners play an important role in policy development and implementation. To begin, they collaborate with other healthcare practitioners to develop policies to guide the operation of healthcare facilities. Also, they collaborate with elected officials to shape policy creation at all levels: local, state, and national. Moreover, they partner with nursing associations to impact the development of national policies.
Capstone Project Outcomes Versus Initial Predictions
As we wrap up, I will assess whether my capstone project met my expectations. The project’s outcomes resonate with my initial prediction. To begin, I aimed to develop an individualized holistic care plan to optimize the symptomatic treatment of Alzheimer’s disease. The patient and her spouse agreed that the intervention would enable her to improve the management of her disease. They also stated that they would embrace the intervention because they were actively involved in its development. Additionally, I anticipated using healthcare technology in my capstone project. This was accomplished through the use of telehealth technologies. According to the patient and her spouse, they are ready to leverage telehealth to improve the management of AD.
My intervention can be considered a best practice. As earlier stated, it comprises the use of medication as well as lifestyle changes. Galvin, Aisen, Langbaum, Rodriguez, Sabbagh, Stefanacci, Stern, and Vassey observed in 2021 that medication is adequate for the symptomatic management of AD. The best results are related to timely treatment commencement and medication adherence. By incorporating telehealth, my intervention encourages and promotes access to healthcare and medication adherence. The proposed intervention is generalizable. It can be used to manage people with AD in other healthcare setups. Care coordination and multidisciplinary teamwork are necessary for the intervention to be applied to a wide patient population. In addition, resources must be allocated to facilitate the training of clinicians, patients, and their families about the effective use of telehealth technology. I spent four hours with the patient and her spouse. They expressed a willingness to actively participate in patient care. The patient and her spouse reported that their daughter would be involved in her treatment to facilitate better adherence to the intervention.
Personal and Professional Growth
As we conclude, I will assess my personal and professional development, not leaving out the areas I am most proud of. The capstone project allowed for personal development. For starters, it allowed me to advance my research skills, which I’m very proud of. For example, I learned how to find credible evidence using online databases and websites. Furthermore, I learned how to evaluate the credibility of the evidence. I also learned that the efficacy of research is impacted by the principles of evidence-based practice. Secondly, I enhanced my interpersonal abilities. In this situation, I used therapeutic communication to build rapport with the patient and her spouse.
Concerning professional development, I adopted nursing ethics and the standards of nursing practice. I safeguarded beneficence and non-maleficence by developing the best evidence-based intervention and ensuring that the potential benefits outweighed the potential risks. I safeguarded the patient’s autonomy by involving her and her spouse in the formulation of the intervention. I safeguarded justice by avoiding stereotypes, bias, and discrimination against the patient and her spouse. I upheld the standards of nursing practice by incorporating the nursing process into the formulation of the intervention, prioritizing quality care, and adhering to current practice recommendations.

For this assessment, you’ll present your approved intervention to the patient, family, or group and reflect on various aspects of your capstone practicum experience. Such reflection will give you a chance to discuss elements of the project of which you are most proud and aspects of the experience that will help you grow in your personal practice and nursing career.

Present your approved intervention to the patient, family, or group. Plan to spend at least 3 practicum hours exploring these aspects of the problem with the patient, family, or group. During this time, you may also consult with subject matter and industry experts of your choice. Be sure you’ve logged all of your practicum hours in the Capella Academic Portal.
