The National Database of Nursing-Sensitive Quality Indicators

Our facility is a top medical destination in my state and is committed to collecting and submitting data to NDNQI (National Database of Nursing-Sensitive Quality Indicators) to fulfil our professional roles as nurses. The NDNQI is a national database for nursing practice. An article by Kaddourah, Abu-Shaheen, and Al-Tannir in 2018 reported a worldwide shortage of nurses, highlighting the need to understand factors contributing to nurse turnover is critical. Nurses form the most significant fraction of healthcare professionals and execute most responsibilities in patient care. An article by Lockhart (2020) on strategies for reducing turnover reported that nursing turnover in the United States is over 19%, with about 33% of nurses living within two years of employment. Accordingly, our facility has not been spared either. At the peak of the COVID-19 pandemic, our facility experienced high turnover rates. At that time, the nurse-to-patient ratio was relatively high, and many of our remaining nursing personnel experienced increased workload and job-related stress. The facility also incurred high costs in replacements. Training new employees on the standard operating procedures of the hospital is a long and expensive undertaking. According to Lockhart (2020), the cost of replacing one clinical nurse ranges between $40,000 and $65,000, which translates to around seven million dollars every year.

Relevance of Monitoring Nurse Turnover

Furthermore, an article by Gebregziabher in 2020 reviewed the turnover theory, developed by Mobley in 1977, which suggested that the intention to leave begins with a feeling of job dissatisfaction, and thus, employees seek alternative work. It is a manifestation of permanent withdrawal behaviour by employees from the workplace. Collecting reliable data on nurses’ experiences at the workplace helps to generate associations between different work dynamics and the intention to leave. Our nurses participated in a survey to determine the factors contributing to the high turnover rates. High nurse turnover negatively affects the ability of an organization to adequately meet patient needs and offer high-quality care. Our chief nurse used the data from this survey to develop measures to reduce nurse turnover in the facility. Notably, an article by Hayat Khan et al. in 2020 identified and classified the factors for turnover and identified the need for multidisciplinary collaboration in diagnosis and policy development to minimize voluntary turnover rates. Therefore, your input as nurses is valuable in transforming the practice and ensuring high-quality patient care at the facility and across the country.

So, how can you be involved in NDNQI?

Nurses are uniquely placed to shape the healthcare system and undertake change control and implementation to improve quality and safety in the future. Our responsibility is to support, engage or disagree with changes in the healthcare sector. As essential members of the multidisciplinary, we are involved in the National Database of Nursing-Sensitive Quality Indicators by data collection and reporting to the database. In the facility, we have frequent surveys to provide feedback about the working environment and engage in policy formulation on leave days, total remuneration, performance appraisal, and other critical factors in practice. The chief of medical services, Dr. Ann Markel, encourages nurses to always submit their feedback through these surveys, which are usually easy to complete. The Role of Nurses in Supporting Accurate Reporting and High-Quality Results

So finally, I would like to share how our actions as nurses are instrumental in impacting the entire healthcare industry. Completing surveys and giving feedback through other formal and informal channels at the facility is vital. Our nurse leaders, through NDNQI, use data from over 1000 health organizations in the U.S to propose operative and traditional improvements and to monitor and evaluate impacts on policy and human resources. Accordingly, these data compare evidence-based approaches between different healthcare sector players and provide a foundation for modifications and transformation. This will ensure that our working environment is transformed to enhance health workers’ self-esteem, patient safety, and organizational efficiency.

The American Nursing Association (ANA) established the National Database of Nursing Quality Indicators (NDNQI®) in 1998 to track and report on quality indicators heavily influenced by nursing action.

NDNQI® was established as a standardized approach to evaluating nursing performance in relation to patient outcomes. It provides a database and quality measurement program to track clinical performance and to compare nursing quality measures against other hospital data at the national, regional, and state levels. Nursing-sensitive quality indicators help establish evidence-based practice guidelines in the inpatient and outpatient settings to enhance quality care outcomes and initiate quality improvement educational programs, outreach, and protocol development.

The quality indicators the NDNQI® monitors are organized into three categories: structure, process, and outcome. Theorist Avedis Donabedian first identified these categories. Donabedian’s theory of quality health care focused on the links between quality outcomes and the structures and processes of care (Grove et al., 2018).

Nurses must be knowledgeable about the indicators their workplaces monitor. Some nurses deliver direct patient care that leads to a monitored outcome. Other nurses may be involved in data collection and analysis. In addition, monitoring organizations, including managed care entities, exist to gather data from individual organizations to analyze overall industry quality. All of these roles are important to advance quality and safety outcomes.

The focus of Assessment 4 is on how informatics support monitoring of nursing-sensitive quality indicator data. You will develop an 8–10 minute audio (or video) training module to orient new nurses in a workplace to a single nursing-sensitive quality indicator critical to the organization. Your recording will address how data are collected and disseminated across the organization along with the nurses’ role in supporting accurate reporting and high quality results.
