The Setting and Access to Potential Subjects

The proposed project will be in a hospital setup. To begin with, the target population for the project is adults in inpatient care. The project will primarily be grounded in the hospital setting. The study participants will be patients admitted into inpatient care for the management of various chronic illnesses. These patients will be assessed progressively to ascertain their hospital readmission status, incorporating aspects such as the frequency of hospital readmission and causal factors for the readmission.  The potential subjects will be accessed from healthcare institutions. An informed consent will also be obtained from the research participants, in line with the provisions of ethical human research. Timeline of the Research

The project will be stratified into three main parts. The first part encompasses all approval processes. Patients will be recruited into the project, and informed consent obtained. The second part of the project encompasses the actual implementation of the project. In this step, patients under inpatient care, transitioning to home-based care, will be assessed to ascertain whether care transition programs were implemented on them. The final part will be to tabulate the results, evaluate the process, and disseminate the findings. Overall, the project is expected to last between 8-9 months. The approval process and subject recruitment are expected to last between one and two months. The actual implementation of the project will last six months, while the tabulation, evaluation, and findings dissemination are expected to last between one and two months.

The Budget and Resource List

The project, in its entirety, fetches considerable financial considerations. It is estimated that the project may require upward of 100,000 dollars. In the proposed project, the methods will allow the establishment of whether the transition care programs should be employed in the comprehensive management of patients with chronic illnesses. Survey forms will be used to collect data on hospital readmission and the likely causes of the readmission 30 days post-discharge. This will allow the researchers to have sufficient data on the frequency of readmission and the underlying causal factors for the readmission.

Process for Delivering the Intervention

The proposed intervention is expected to be delivered to all stakeholders impacted by the plan. The delivery process will employ an educational approach targeting all the stakeholders. Patients will be educated on the project, including how the findings may impact the care delivered to them and the significance of their participation in the study. The hospital administrators and caregivers involved in chronic care will also be notified of the study and its implications for chronic care.

Stakeholders Needed

The stakeholders needed for the effective implementation of the plan include the hospital administration, caregivers involved in chronic care, the patients, the university library, and the university administration. The university and hospital administrations are responsible for providing the required approvals and permits. Caregivers involved in chronic care are drawn into the plan as they are the primary implementers of care transition programs. The patients, on the other hand, are the research subjects.

The Implementation Plan

The proposed project seeks to evaluate the effectiveness of care transition programs in lowering hospital readmission. A potential challenge for the project borders on its financial considerations. The project may be costly as it may involve a large sample size. To overcome this challenge, finances will be mobilized through donation requests from the research department and the university library. These institutions may help in fund mobilization, as well as directly provide the funds required for the project.

The implementation plan remains feasible. The required resources, coupled with the project’s time frame, allow for its successful implementation.
