In my clinical practice, I have had extensive experience working with both elderly patients aged 65 and older, as well as children and adolescents with chronic health conditions. Caring for older adults often involves managing multiple comorbidities, ensuring medication adherence, and addressing mobility, cognition, and social support issues, as indicated by Abdi et al. (2019). Further, my work with children and adolescents with chronic conditions, such as asthma, diabetes, and congenital heart disease, requires a multidisciplinary approach to ensure these young patients receive comprehensive care tailored to their unique developmental needs.

One specific technology platform I have found particularly useful in managing chronic conditions is the smartphone application MySugr, developed by Roche Diabetes Care. MySugr is designed for individuals with diabetes to track their blood sugar levels, carbohydrate intake, insulin doses, and physical activity (Kebede & Pischke, 2019). The app offers a user-friendly interface where patients can log their daily data, view trends, and generate reports to share with their healthcare providers. MySugr integrates seamlessly with various glucose meters, reducing the manual input required and improving the accuracy of the data collected.

The current price of MySugr varies; the basic version is free, but there is a subscription fee of approximately $2.99 per month or $27.99 per year for the Pro version, which offers additional features such as detailed reports and personalized feedback (Aguillard & Garson Jr, 2020). A major pro of MySugr is its ability to empower patients through self-management by providing real-time feedback and motivational elements, such as challenges and rewards, as stated by Ehrmann et al. (2022). However, a notable con is that some users may find the subscription cost prohibitive, and the reliance on technology may be challenging for patients who are not tech-savvy or lack access to smartphones.
