Care Coordination Presentation to Colleagues
Hello and welcome to the West District Hospital. This hospital maintains excellence in delivering quality healthcare services to its clients. Through coordinative approaches utilizing collaborative paradigms, the hospital has been successful in the effective management of chronic diseases such as hypertension. In this presentation, we will discuss the fundamentals of care coordination with an emphasis on effective collaborative strategies, aspects of change management that directly impact the elements of patient experiences, coordinative approaches in ethical decision-making, and the impacts of healthcare policies on the coordination and continuum of care. But first, let us detail what care coordination is.
Care coordination is central to all care operationalization processes in healthcare. The Agency for Healthcare Research and Quality defines care coordination as the deliberate organization of all patient activities among all individuals involved in caregiving. It sums all caregiving roles on the patients and synchronizes them to not only bridge apparent gaps in care but uphold the effectiveness of care provided. To enhance their effectiveness in coordinating care, nurses should have a vast knowledge of best practices in care collaboration that will enable them to engage their patients and the patient’s families to optimize care approaches.
Effective Strategies for Collaborating with Patients and their Families to Achieve Desired Health Outcomes
Several strategies can be used to enhance the effectiveness of caregivers’ engagements with their patients and patients’ families. Effective strategies for collaborating with patients and their families include patient education, self-management support, and shared decision-making (Menear et al., 2020). Shared decision-making implores caregivers to involve their patients in essential decision-making processes on healthcare interventions targeted to them. It forms the groundwork for future engagements by ensuring healthier therapeutic relationships between the patients/families and their caregivers and enables them to be on board with the therapeutic plan designed for them.
Patient education is another effective strategy for engaging patients and their families. The management of hypertension and other chronic disorders requiring long-term therapeutic plans benefits considerably from these strategies. Educative approaches enable caregivers to enhance their patient’s capacity to maintain accountability over their disease process. Through these processes, caregivers can educate their patients on the significance of medication compliance, anticipated side effects of the medication they are taking, how to address them, and when to seek medical help. Grau Canét-Wittkampf et al. (2020) report that educative approaches in chronic care set the management trajectory toward optimizing clinical outcomes by ensuring closer engagements between patients and their caregivers. It is thus important that nurses be conversant with this collaborative strategy.
The Aspects of Change Management that Directly Affect Elements of the Patient Experiences
AHRQ details the significance of patient experiences in comprehensive care for chronic disorders. Understanding patients’ experiences is central to patient-centered care as it enables the tailoring of care services to meet the specific demands of the patients. Several aspects of change management have a direct impact on patients’ experiences. According to Errida & Lotfi (2021), leadership alignment to the change process, effective communication between organizational elements and between caregivers and their patients, the impact of the change process on care operationalization and clinical outcomes, and the readiness of caregivers and the patients for the change process will directly impact various aspects of patients experiences. While organizational leadership inspires change processes in organizational members, members’ preparedness for the change ensures a greater buy-in to the change process. Communication helps in readying organizational elements to the change process. It is thus imperative that organizational leadership and other stakeholders within the care organization work concertedly in implementing change processes that will directly impact patient experiences.
Positive patient experiences have been implicated in better clinical outcomes, higher patient satisfaction ratings, and improved clinical safety. Integral in quality care is the optimization of change processes that enhance patients’ experiences most valued by the patients. Hirpa et al. (2020) report that patients highly value easy access to health and healthcare information, good communication between caregivers and their patients, and efficiency in getting appointments. Ensuring patient satisfaction is also key in the overall maintenance of quality care. While patients experience detailed interactions between caregivers and their patients, patient satisfaction is the subjective measure of care services that often correlates with the patient’s expectations of care services they anticipate during contact with caregivers.
The Rationale for Coordinated Care Plans Based on Ethical Decision Making
Ethical decision-making in healthcare implores caregivers to utilize ethical provisions in their decision-making processes. The ethical principles of beneficence, autonomy, non-maleficence, and justice guide clinical decision-making processes and may help caregivers navigate ethical complexities. These provisions guide clinical decision-making processes and interactions to ensure best practices are utilized in designing therapeutic plans.
The consequences of ethical complications are often evident in improved clinical outcomes, better patient satisfaction, and enhanced patient experiences. This is mostly because ethical decision-making implies that best clinical practices are utilized to meet the needs of specific patients. Of importance during ethical decision-making is the diversity of the patient population presenting for care. An assumption during ethical decision-making is that each patient is willing and ready to cooperate with the caregivers in designing their therapeutic plans.
The Potential Impact of Specific Health Care Policy Provisions on Outcomes and Patient Experiences
Several healthcare policies influence healthcare administrations and may consequently affect clinical outcomes and patient experiences. The Health Information Technology for Economic and Clinical (HITECH) Act is an example of a healthcare policy that has a significant impact on clinical outcomes and patient experiences. This policy promotes the implementation of meaningful health information technologies in care operationalizations. Health technologies such as telehealth maintain significance in bridging communication gaps in healthcare that result from proximity. This makes the HITECH provisions important in the overall care provision processes.
The provisions of HITECH have a considerable impact on several aspects of patients’ experiences. Lite et al. (2020) report that the HITECH Act has significantly improved the security and privacy of electronic PHI through EHR and enhanced healthcare communications and delivery. The overall consequence of these provisions is often evident in improved patient trust in the care services and satisfaction with the care processes.
Nurse’s Vital Role in the Coordination and Continuum of Care
Nurses play an essential role in coordinating care processes for patients with chronic disorders. Their knowledge of the disease processes, coupled with their roles in patient advocacy and education, makes them invaluable in addressing patients’ needs and leveraging other organizational resources to help their patients. We should, therefore, maintain and enhance our knowledge of coordinative approaches in healthcare and how we can utilize our nursing knowledge to influence aspects of patient experiences.
In conclusion, care coordination is integral to quality maintenance and enhancement approaches in care. Caregivers should thus know various collaborative strategies that enable care coordination. Significant in care coordination are aspects of change management that foster patient-centeredness by enhancing patient experiences. Also important in the pursuit of coordinated care are the policies that affect care operationalization and delivery.

Care Coordination Presentation to Colleagues
Introduction
Nurses have a powerful role in the coordination and continuum of care. All nurses must be cognizant of the care coordination process and how safety, ethics, policy, physiological, and cultural needs affect care and patient outcomes. As a nurse, care coordination is something that should always be considered. Nurses must be aware of factors that impact care coordination and of a continuum of care that utilizes community resources effectively and is part of an ethical framework that represents the professionalism of nurses. Understanding policy elements helps nurses coordinate care effectively.
This assessment provides an opportunity for you to educate your peers on the care coordination process. The assessment also requires you to address change management issues. You are encouraged to complete the Managing Change activity.
Completing course activities before submitting your first attempt has been shown to make the difference between basic and proficient assessments.
Preparation
Your nurse manager has been observing your effectiveness as a care coordinator and recognizes the importance of educating other staff nurses in care coordination. Consequently, she has asked you to develop a presentation for your colleagues on care coordination basics. By providing them with basic information about the care coordination process, you will assist them in taking on an expanded role in helping to manage the care coordination process and improve patient outcomes in your community care centre.
To prepare for this assessment, identify key factors nurses must consider to effectively participate in the care coordination process.
You may also wish to:
Review the assessment instructions and scoring guide to ensure you understand the work you will be asked to complete.
