It requires prior knowledge about the needs of the patients and timely communication of these needs. Care coordination achieves a better quality of healthcare services provided to the patient.

Effective Strategies for Collaborating with Patients and their Families

Allow me to address the strategies for collaborating with patients and families. Various strategies can be used to promote collaboration between healthcare providers, patients, and their families. This collaboration is essential because it ensures that all patients’ needs are addressed.

The first strategy entails open communication. (Institute for Patient and Family-Centered Care (IPFCC), n.d.) reports that healthcare workers should inform the patients about the vision and values of the healthcare facility and their impact on the treatment process. The mission, vision and values should be aligned to support patient and family involvement. IPFCC (n.d.)  reports that this is achieved by creating purpose and identifying priorities to be fulfilled. Furthermore, open communication of the mission, vision, and values incorporates the patients and their family members into the strategic plan of the healthcare facility. This increases the likelihood of achieving the desired outcomes.

The second strategy by assuming the role model capacity that promotes patient involvement. IPFCC (n.d.) reports that this can be accomplished by involving patients and their families in leadership rounds. An example is involving patients in determining whether medication is administered at the prescribed dosing interval. Subsequently, this makes the patients and their families recognize that they are important team members in the care process. Additionally, patients can be involved in the formulation of procedures and policies. This can be accomplished by ensuring that new policies are not implemented until the perspectives and opinions of the patients and their families are considered. By so doing, culturally sensitive policies and procedures will be developed and improve patients’ access to quality healthcare services.

Aspects of Change Management

At this point, I will discuss the aspects of change management that impact patients’ experiences and are key to providing quality patient-centered services. The components of change management can affect patients’ experiences when accessing quality and patient-centered services. Leadership alignment is the first aspect. This entails identifying leaders and potential sponsors of a project or plan and harmonizing their objectives and interests. Writing in 2018, author Popa reports that leadership alignment ensures that the other stakeholders, including patients and other healthcare workers, understand the specific goals and objectives of the treatment plan. This increases the likelihood of quality service delivery to patients.

The other aspects are stakeholder engagement and communication. Stakeholder engagement entails creating a plan of collaborating with relevant stakeholders, including patients. In this context, active patient involvement makes them appreciate their importance in the treatment process. Bombard, Baker, Orlando, Fancott, Bhatia, and Casalino (2018) reported that stakeholder engagement allows patient autonomy and culturally sensitive services to be provided. Communication requires establishing open communication between patients and the other members of the interdisciplinary team. This creates an atmosphere of trust and respect. Furthermore, it acts as an avenue for holistic nursing services because the patient’s preferences are considered. By so doing, the patients, as key stakeholders, access quality patient-centered services.

The Rationale for Coordinated Care Plans Based on Ethical Decision Making

Allow me to discuss the rationale for coordinated care plans based on ethical decision-making. Tseng and Wang (2021) reported that ethical decision is used to address conflicts and uncertainties that accompany moral values. Ethical decision-making is required to make desirable judgments when a healthcare provider is faced with a moral dilemma. Accordingly, ethical decision-making is relevant when establishing coordinated care plans. An ethical decision-making process can embrace two major philosophical approaches: utilitarian and Deontological ethics. Accordingly, Tseng and Wang (2021) reported that utilitarian ethics direct healthcare providers and clinicians to engage in activities that are beneficial or do good to the general population. This is consistent with the ethical principles of beneficence and non-maleficence that advocate for actions that benefit patients and do not harm patients, respectively, as reported by McDermott-Levy, Leffers, and Mayaka in 2018. Utilitarian ethics will ensure the formulation of coordinated care plans that benefit the patients.

Moreover, Tseng and Wang (2021) reported that deontological ethics directs healthcare providers and clinicians to respect people and treat them with dignity. This is consistent with the ethical principle of autonomy that respects the decisions of the patients regarding their treatment plans. Implementing deontological ethics will ensure that the coordinated care plans are culturally sensitive and provide holistic nursing services.

The Potential Impact of Specific Health Care Policy Provisions

At this point, I will describe the impact of the provisions of a specific healthcare policy on patient outcomes and experiences. The Affordable Care Act (ACA) is an example of a policy that affects patient outcomes and experiences. The ACA has various goals. The first goal is to make affordable health insurance available to a larger population. Notably, Michener (2020) reported that the second goal is to expand the Medicaid program and cover adults whose income is less than 138% of the federal poverty level. The third goal is to support innovativeness in healthcare delivery methods and lower healthcare costs.

The ACA has ensured that more eligible people living in the USA are insured. Accordingly, there has been increased access to healthcare services by this population. In addition, Michener (2020) reported that in 2019, about 11 million Americans were enrolled for coverage via ACA. Increased access to quality healthcare services is associated with better patient outcomes and experiences. This has been particularly relevant for the patient population with low incomes. Additionally, the ACA benefits the disabled patient population through its consumer protections. However, it should be noted that ACA minimized access to care for those not eligible. Worth noting is that Michener (2020) reported that the affected group includes illegal immigrants, those with a gross income above 400% of the federal poverty level, and those who can access comprehensive coverage provided by their employers.

Creating Awareness of Nurse’s Vital Role in the Coordination and Continuum of Care

As we edge toward the end, I will identify the role of nurses in the coordination and continuum of care. Nurses are important interdisciplinary team members in developing coordinated care plans and ensuring a continuum of care. Their first role is embracing evidence-based practices when administering care to the patients. These practices are based on research and are harmonious with the utilitarian and deontological ethical provisions. In 2018, authors Bombard, Baker, Orlando, Fancott, Bhatia, and Casalino reported that evidence-based practices help to increase the quality of service delivery and achieve better patient outcomes. The second role involves evaluating patient outcomes. This can be done using quarterly and annual reports from the National Database of Nursing Quality Indicators, as Oner, Zengul, Ivankova, Karadag, and Patrician reported in 2021. This helps to facilitate goal-based coordinated care with other interdisciplinary team members to improve underperforming nursing-sensitive indicators. By so doing, patient safety and better outcomes are achieved.

Further, nurses promote coordination and continuum of care by advocating for appropriate resource allocation and relevant policy formulation. Writing in 2018, authors Hannigan, Simpson, Coffey, Barlow, and Jones reported that resource allocation entails the optimization of nursing staff ratios and the acquisition of necessary medical equipment and protective equipment. Formulating policies that advocate for interdisciplinary collaboration ensures that the quality-of-service delivery is enhanced. This is because of the interdisciplinary decision-making process.

In conclusion, care coordination refers to the organization of patient care practices and open communication among the stakeholders involved in patient care to realize effective and safe patient care. Patient and family engagement can be achieved by open communication. Nurses are key members in the coordination and continuum of care.
