Learning the ways in which evidence-based models and care can help remote work produce better outcomes will become critical for success. Additionally, understanding how to leverage EBP principles in collaboration will be important in the success of institutions delivering quality, safe, and cost-effective care. It could also lead to better job satisfaction for those engaging in remote collaboration.

Remote care and diagnosis is a continuing and increasingly important method for nurses to help deliver care to patients to promote safety and enhance health outcomes.   • Review the case study in the Assessment 04 Supplement: Remote Collaboration and Evidence-Based Care [PDF] resource. You will use this case study as the focus for this assessment.

Instructions

For this assessment, you are a presenter! You will create a 5–10-minute video using Kaltura or similar software. In the video:

  • Propose an evidence-based care plan that you believe will improve the safety and outcomes of the patient in the case study presented in the Assessment 04 Supplement: Remote Collaboration and Evidence-Based Care [PDF] resource. Add your thoughts on what more could be done for the patient and what more information may have been needed.
  • Discuss the ways in which an EBP model and relevant evidence helped you to develop and make decisions about the plan you proposed
  • Wrap up your video by identifying the benefits of the remote collaboration in the scenario, as well as discuss strategies you found in the literature or best practices that could help mitigate or overcome one or more of the collaboration challenges you observed in the scenario.

With the advancement in technologies and in the field of healthcare, telepractice, telehealth, and telecommunication are now typical ways through which a nurse is expected to practice. It is important to understand how the EBP models and care can enhance the results of remote work in order to succeed in it. EBP principles also have a great contribution to promoting collaboration because healthcare institutions can offer high-quality, safe, and cost-efficient care. Knowledge of the advantages and disadvantages of remote working and applying the EBP models as a nurse will enable me to deliver efficient patient-centered care that has the potential to enhance patients’ health status.

Case Study: Addressing Anxiety in a Rural Setting

The case under discussion presents a 40-year-old female patient living in a rural area, suffering from severe anxiety and hypertension, and having no access to mental health care providers. The patient described having an anxious depressive mood that was manifested by excessive worrying, agitation, and difficulty concentrating, which affected her daily functioning. Anxiety also leads to hypertension, which is another issue with this patient. She has to take care of her elder parents besides taking responsibility for her own family, which has led to more stress. Remote collaboration provides an opportunity to address these issues as this format gathers medical staff and materials that are hardly accessible to a patient.

Application of the Stetler Model in Care Plan Development

The Stetler Model of evidence-based practice was instrumental in developing this care plan. This model emphasizes critical thinking and the use of research findings to create practical, patient-centred solutions. In Phase I, I identified the priority need: reducing the anxiety and hypertension of the patient through simple home-based telemanagement. Phase II aimed at corroborating findings from other scholarly works proving the efficacy of CBT and telehealth in addressing anxiety. In Phase III, I critiqued the evidence, taking into account the applicability of the proposed interventions in a rural context. In the final phase of the process, I developed a plan of care where I described the plan for implementing the changes and assessing the results. Lastly, in Phase V, an assessment of the outcomes will be completed to establish the success of the plan in the reduction of the patient’s anxiety and high blood pressure (Dusin et al., 2023).

Using the Stetler Model, it was easier to create a framework for developing a structure for implementing the changes and ensure that each process is rooted in the available literature and formulated in a way that addresses the patient’s needs. This model clearly shows that by following these guidelines, the care plan would help the patient receive appropriate interventions and increase her well-being.

Benefits of Remote Collaboration in Healthcare

Telemedicine is a valuable concept since it enables care delivery in areas with limited access to physicians. It enables care coordination across various health professionals, including psychiatrists, medical practitioners, and nutritionists, among others, without the need for the patient to travel far. In this case, remote collaboration is particularly appropriate because the patient can receive specialized care and tools that are not available locally. This approach also increases patients’ access to care and increases the quality of patient care through interprofessional collaboration and information sharing (Gajarawala & Pelkowski, 2021). In addition, the use of remote collaboration can enhance the interaction and perception of the patients by offering them ideas that are more flexible, convenient, and personalized.

One challenge is the communication of the healthcare team members since communication is central, especially in Health facilities. Another issue to consider is a lack of access to the Internet or the absence of a tablet or other digital device. These technical factors hinder communication and minimize the effectiveness of remote assistance. To overcome these challenges, the communication channels should be well prescribed, and the right use of efficient telecommunication should be ensured in order to enhance the interaction of the members of the group. Alleviating connectivity-related issues and supporting patients and healthcare staff with technical assistance in connectivity results in a better overall remote care experience (Rawlinson et al., 2021).

Another issue that tends to arise is the issue of patient rights and their ability to keep their information private, especially when accessing services from remote locations. Enumerating privacy measures and getting their consent can also help to ensure patients’ trust and compliance with the legal and ethical requirements when engaging in remote communication. Furthermore, the concept of healthcare security requires SHAs and other healthcare providers to closely supervise possible security threats because all related communication needs to be protected and performed safely.

Conclusion: Enhancing Patient Outcomes Through Remote Collaboration and Evidence-Based Practice

Finally, I have determined that telepractice and EBP are efficient ways to enhance patient outcomes, especially in rural healthcare facilities. This paper has demonstrated that through the use of frameworks like the Stetler Model and remote interventions, including telehealth CBT sessions, healthcare providers can meet the challenging needs of their patients while providing the best quality patient-centered care. Although there is always some level of resistance, such as communication barriers and privacy issues, these major challenges can be managed through proper planning, the use of technology, and sticking to recommended processes and practices. Finally, remote collaboration improves patients’ access to care, promotes cross-disciplinary work, and engages individuals with the management of their care and treatment, resulting in better health outcomes and well-being.

The two concepts of remote care and evidence-based practice can help transform a health system to be more patient-focused, less location-bound, more responsive to patient needs, and more efficient.
