The clinical system of focus in this literature review is electronic health record (EHR). An electronic health record is information related to the healthcare, health, and wellness of a person in computer-processable type and signified, conferring to a consistent information form that is interoperated and shared around healthcare settings and is patient-centred. The electronic health record was introduced to increase productivity, efficiency, and patient care quality. This clinical system has, over time, evolved and further developed to address challenges such as data duplication while achieving its purposes at the same time. This paper annotates four peer-reviewed articles that examined how electronic health records have increased efficiencies and outcomes in healthcare settings.

The study was aimed at exploring if there is an impact on communication and interaction quality when nurses use electronic health records. Data collection was done by extracting data from previous research that ticked the inclusion criteria involving the methods, characteristics, and evidence strength. The studies used examined the communications and interactions between a patient and nurse while electronic health records were still in use in the particular healthcare setting. Afterwards, a thematic approach was used to synthesize the findings. Out of the 1,920 articles, eight passed the inclusion criteria. The thematic analysis identified four significant themes showing that electronic health records improve formulaic and task-oriented communication, obstruct one-on-one communication, and affect communication pattern types. The study concluded that electronic health records encouraged locked nurse-patient communications and promoted a task-oriented approach. The study aimed to examine the economic influence of electronic health record-oriented clinical decision-support interventions. Also, the researchers examined the best practice approach for the specific interventions based on the cost-outcome (Lewkowicz et al., 2020). Part of the study goal was to examine cost-effective use fields for various medical risk factors. The study involved conducting a systematic literature review on studies based on the economic benefits and impact. The clinical decision support interventions explored include medication dosing support, workflow support, expert systems, point-of-care reminders, and order facilitators (Lewkowicz et al., 2020).
27 studies were used in the review, with 22 studies showing that after the implementation of the electronic health record, healthcare expenditure was reduced. Healthcare greatly reduced specific application areas like unnecessary lab tests, order entry duplication, effective transfusion practices, or decreased antibiotic prescriptions. The study noted that the positive economic effect of electronic health records grounded on clinical decision-support interventions is extremely promising (Lewkowicz et al., 2020).

The electronic health records integrate the computerized provider order entry systems and clinical decision support interventions. Data was collected by using focus groups through interviews with reports from consultants and clinic leaders to examine the implementation process and the purpose of the electronic health record. The findings of the study were that all clinics showed an overall increase in person-centred care preparation fidelity at the end of the study period (Stanhope & Matthews, 2019).
The use of electronic health records encourages efficient person-centred care planning. The functioning of the clinical system and person-centred practices delivery shape each other. Person-centered care is communicated, promoted, and reflected by the service plan which entails both the treatment particulars and spirit. Among the purposes of electronic health records, is to promote collaboration between healthcare professionals. The study aimed at examining how electronic health records nurture and constrain collaboration in healthcare settings. Health professionals’ collaboration is key to achieving high-quality patient care, sharing skills and knowledge, and integrating information (Vos et al., 2020). The study collected data through the use of semi-structured face-to-face interviews. The data was analyzed by transcribing, generating codes, and then themed. The study findings showed that electronic health records help health workers coordinate patient care at any time and place, making it very flexible. Also, the clinical system reduced time wastage by reducing face-to-face communication (Vos et al., 2020).
The electronic health record has been helpful in the improvement of outcomes and efficiencies in the healthcare setting through facilitating collaboration. According to research, healthcare professionals have different goals. However, the electronic health record helps streamline the goals into a common objective, which encourages collaboration (Vos et al., 2020). Additionally, the electronic health record provides affordances for supernumerary healthcare providers, role-shifting, access, usage, and capture of patients, as well as for monitoring, coordinating, integrating, and standardizing care. It also enables quick information utilization in clinical decision-making. Moreover, this particular clinical system has shown a quality that nurtures collaborative action and planning among different actors (Vos et al., 2020).
In conclusion, this paper is a systematic review that used different studies to show the impact of electronic health records in the healthcare setting. Electronic health records have proven to be a valuable tool in increasing efficiency and outcomes in the health setting. As mentioned above, this clinical system is essential for the improvement in nurse-patient interaction, promotion of inter-disciplinary collaboration, cost-effectiveness promotion, and enhancement of patient-centred care. Through incorporation with other systems, like the computerized provider order entry systems, the electronic health record turned out to be a valuable clinical system to improve patient care and obtain positive outcomes.
