These made me learn how advanced practice nurses support and participate in the campaign of public health, support health reforms, and fight for policy reforms in the health sector. Lessons Learned from AACN Advanced Level Domain 3 Competencies
Learning about the AACN Advanced Level Domain 3 competencies broadened my knowledge of the advanced professional nurse’s role in population health management. This domain addresses fair and equitable health care, policy advocacy, and inter-professional collaboration to address health disparities. A key takeaway was understanding how nurses influence healthcare policies so that underserved populations receive appropriate healthcare.
Population health management requires proficiency in developing policies, evidence-based practices, and leadership for health care reform and improved outcomes for the patient. I also gained insight into using data analysis and healthcare informatics to examine the effectiveness of healthcare policies and interventions so adjustments can be made to address the population’s needs better.
I also understood the importance of collaboration with public health officials, policymakers, and health professionals to overcome barriers to care and social determinants of health. Nurses must be actively involved in population health practices that are prevention-centered, health promotion-oriented, and provide better access to care. Competences also stress health equity and culturally competent care so that varied populations are included in the policymaking process.
Understanding these concepts reinforced my determination to work towards policies ensuring patient-centered care, eradicating disparities, and expanding access to health care. This research solidified my belief that nurses should not only give direct care to patients but also be leaders in forming healthcare policies.
Steps Towards Becoming Actively Involved in Health Policy Advocacy
To further enhance my policy advocacy efforts, I plan to engage in forums and discussions related to community health. Such forums will inform me of local health problems and help address them through policies. By interacting with public health leaders and health professionals, I can identify areas for improvement and encourage preventive care programs that reduce hospitalization rates.
Engaging in such forums allows me to meet with stakeholders and decision-makers who decide on local, state, and federal health care. Direct engagement with local community leaders will provide effective and practical solutions towards healthier outcomes.
I also plan to join professional bodies like the American Nurses Association (ANA) so that I can have access to policy information and advocacy groups. Active involvement in such groups will allow me to stay abreast of changes in legislation and future health issues. I also plan to attend forums on policies and advocacy meetings to learn about the process of legislation and ways of interacting with policies.
I will also utilize social media and web-based platforms to raise awareness of health policies, disparities, and the role of nursing advocacy. By spreading credible information and interacting with the public through forums, I can reach more individuals and influence public perception of health issues.
Positive Peer Feedback on Videos
Tara’s presentation was organized and engaging. She illustrated the role of nurses as change makers and advocates for change about equity in health. She was thoughtful in her focus on preventive health and mental health. Providing some specific policy initiatives she supports would make her advocacy more concrete.

In this phase, I have understood ways of interprofessional collaboration in the context of population health and how effective teamwork can be in improving care results. I now have a better appreciation of communication, leadership, and collaborative decision-making amongst the members of the interprofessional teams. This reflection will reflect on what I learned about creating interprofessional teams and what I will do as an advanced professional nurse to improve them.
Insights Gained About Forming an Interprofessional Team in Population Health
Engaging in Phase 2 CPE, I understand the importance of collaboration of different health professionals in meeting the population’s health care needs. These teams consist of a varied workforce that includes nurses, physicians, pharmacists, social workers, and specialists in public health to ensure patients get an advanced and patient-centered treatment plan. Every team needs to understand that to work well together, there must be communication, respect, and vision. Interprofessional relationships help strengthen safety and minimize and improve the general healthcare system
Further, I realized that leadership and advocacy are fundamental components of interprofessional teams. Nurses, especially advanced practice nurses, assume different roles, including a patient advocate and coordinator who ensures concerns are considered. Furthermore, interprofessional practice is not only limited to the healthcare systems of hospitals and clinics but is also implemented in health programs operated within the community to target health disparity, chronic disease management, and prevention. When the professionals exploit the strength of each team member, health will be enhanced, and equity will be achieved for the marginalized groups.
Steps to Enhance My Skills in Developing Interprofessional Teams
I must strengthen my leadership and advocacy skills to become a more effective leader in interprofessional collaboration. One strategy is to engage in learning through professional development, targeting team care, health leadership, and interprofessional education. Additionally, I will ask healthcare leaders who have worked with an interdisciplinary team to mentor me, as they might have gained adequate experience in the field. These approaches will assist me in motivating and directing healthcare professionals toward the achievement of organizational goals.
It also entails effective communication and conflict management, other important competencies supporting the forming of professional teams. To improve the aspects of discussion and conflict resolution and my relationships with my colleagues, I plan to pursue advanced courses in communication. Furthermore, I want to participate in interdisciplinary workshops and simulations wherein I can work with other practitioners of different occupations and acquire a practical approach to problem-solving. By developing such skills, I can contribute positively towards an effective working environment within a team.
Constructive Feedback to Peers
From Reys’ video reflection, I found his insights on leadership in interprofessional teams to provide a deeper understanding. He rightly described how a nurse could work to ensure patient-centered care and bring about the necessary changes in healthcare policies. His example of using professional associations to advance change highlighted regular competence in the value of partnership and participation.
Nevertheless, I would like him to elaborate on the ways various professionals in the provision of health care contribute to an interprofessional team. If he were to give real-life examples drawn from his own experiences, his reflection would be much more imposing.
Christian’s explanation of the five stages of group development was very elucidated and comprehensive regarding interprofessional teamwork. His actual case of the telemetry project was well exemplified to explain the issues that teams encounter, especially in the storming phase. To strengthen his observations, he could expand on measures like training the team members, considering conflict resolutions before they occur, and providing other managers to lead new projects to avoid the same problems. Building further on these strategies would enrich his reflection by providing tangible advice about enhancing collaborative work in future projects.
