Explanation of Symptoms
The presence of scalp dandruff, pitting of nails, and joint pain put me under high suspicion for psoriasis with probable psoriatic arthritis. Psoriasis can be defined as an autoimmune disease in which skin cells multiply more rapidly than normal, resulting in a build-up of skin cells, inflammation, and, in some instances, arthritis. Nail pitting is one of the modes of presentation of psoriasis which is said to have a direct link with the intensity of the disease. The morning joint pain of the patient, which worsens at night and eases with exercise, matches the features of psoriatic arthritis, a form of arthritis related to psoriasis that entails joint stiffness and pain that reduces during the day.
The failure of the patient to respond to dandruff shampoos indicates that the dandruff is not a result of a simple superficial fungal infection as in seborrheic dermatitis. Still, it is a part of the inflammatory process of psoriasis. The immune response in psoriasis is primarily driven by T cells, especially Th1 and Th17 cells, which release pro-inflammatory cytokines such as TNF-α, IL-17, and IL-23. These cytokines promote the inflammation and abnormal keratinocyte proliferation seen in psoriasis. Immunosuppressive therapies are therefore essential in managing psoriasis by targeting these specific immune pathways and reducing the aberrant immune activity (Dhabale & Nagpure, 2022).
Nevertheless, immunosuppressive agents affect not only the recipient’s immunity but the organism as a whole. Although it works to suppress troublesome immune function in diseases such as psoriasis, it will also decrease nonpathological immune response, making the body more vulnerable to infections from pathogens. Also, patients on long-term immunosuppression are at an increased risk of certain types of cancer because the immune system, which normally recognizes and destroys tumor cells, is suppressed. The knowledge of genetic and immunological basis facilitates the development of therapy, which helps to enhance the treatment results for patients with psoriasis.
