General Information:
- Name, age, gender, and contact information of the child or youth.
- Name and contact information of parent(s) or guardian(s).
- Date of assessment.
Living Situation:
- Who all live in your home? (Include relationships and ages)
- What’s life like in your family?
- What do you like to do together as a family?
- How would you describe your father, mother, and siblings?
- How do you get along with your father, mother, and siblings?
- Do you have many members in your family or not so many? Do you get to see your aunties, uncles, and grandparents?
School and Activities:
- How are things going for you at school?
- Are you involved in any extracurricular activities or clubs?
- Do you feel safe and supported by your teachers and school staff?
- Tell me about how things work at your house, for example, chores.
- What and when do you usually eat?
- Who usually cooks for you?
- Who takes you to school?
- Where are your parents when you come home from school and when you are doing your homework, waking up, and going to sleep?
- Do you have good days in your house? Tell me what a good day in your house looks like. How about a not-so-good day?
- What takes place on the not-so-good days?
- Does anyone comfort you when you’re having a bad day? Or when your parents are having a bad day?
- How do they comfort you?
Safety and Well-being:
- How do you feel when you are at school or in your community?
- Do you have any friends or adults you trust to talk to about problems?
- Has anyone ever hurt you physically or emotionally?
- Do you feel safe online? Have you ever experienced cyberbullying or inappropriate interactions online?
- Do you sometimes have friends over after school or go over to their houses?
- Who can you talk to if you’re worried or sad? Do they help you feel better?
- Who has been there for you in the past?
- Can you talk to your teacher/coach/friend’s parent?
- Who are the top three people in your life you are closest to?
Support System:
- Who do you go to when you need help or support?
- Are there any trusted adults in your life who you feel comfortable talking to about anything?
- Most kids feel anxious now and then. Can you tell me what you feel anxious about?
- Do you worry about your parents? What kinds of things worry you?
- Who cares for you when your parents are not able to? Who pays attention to you, looks out for you, and cares about what happens to you?
- Who do you go to when you are having a tough time? What do they do to help?
- Have you ever received counseling or therapy? If yes, was it helpful?
Previous Incidents/Types of Abuse:
- Do you know about Child Protective Services? Have you ever been involved with Child Protective Services before?
- Have you ever run away from home or felt like running away?
- Has anyone ever touched you in a way that made you feel uncomfortable or pain? (Physical abuse)
- Has anyone ever said things to you that made you feel bad about yourself? (Emotional abuse)
- Has anyone ever forced you to do something you didn’t want to do? (Sexual abuse)
- Do you feel safe discussing your feelings and opinions at home? (Verbal abuse)
- Are there any adults or older children who make you feel uncomfortable or scared?
Safety Planning:
Do you know what to do if you feel unsafe or threatened?
Are there any specific places you feel safe or unsafe?

After reading Chapter 7 and reviewing the two websites regarding family assessments and safety and risk assessments, I want you to create your own Child Safety and Risk Assessment form.
Think about what questions need to be on the form.
What will you ask the child or youth that will give the needed information to assess whether or not there is a risk of harm?
Think about the different types of abuse you have learned about.
What questions would you ask the child/youth in order to have a thorough assessment? What information do you need to know to make your assessment?
What do you need to ask about in your assessment? (For example: Who all lives in your home? When you get into trouble, what happens? Who disciplines you?)
Think: Who? What? Where? When? How? Why? Create a Word document and upload it into Canvas.
