Treatment of Generalized Anxiety Disorder
Pharmacokinetics And Pharmacodynamics Profiles of Anxiolytic Medications Used to Treat GAD
Pharmacokinetics
Common anxiolytic medications display almost similar pharmacokinetic profiles. Most anxiolytic medication’s mechanism of action involves potentiating gamma-aminobutyric acid (GABA) in the central nervous system (CNS), leading to reduced neuronal excitement and hence quieting CNS nerve activity (Kaur et al., 2020). The absorption, distribution, metabolism, and elimination of the medications vary across the various classes and individual drugs within each class.
SSRIs and SNRIs
SSRIs’ mechanism of action includes the inhibition of the reuptake of serotonin. This results in increased neurotransmitters in the synapse while SNRIs act by blocking noradrenaline and dopamine reuptake. This can also increase serotonin; however, it can be done in higher doses.
Tricyclic antidepressants (TCAs)
TCAs mechanism of action is characterized by pre-systemic first-pass metabolism, which then increases serotonin and noradrenaline in the synapse by blocking the reuptake of both neurotransmitters (López-Muñoz, 2023).
Benzodiazepines
The mechanism of action for most benzodiazepines includes increasing the activity of GABA, creating the therapeutic effect of quieting nerve cell activity. The actual onset of action for this class of anxiolytics varies on individual medications.
Pharmacodynamics
Anxiolytic medications act on various sites on the brain to impact a range of CNS, creating the effects and side effects of the medications. The medications induce varied pharmacological effects through modulation of specific GABA receptors in the brain, while SSRIs cause serotonergic stimulation of 5-HT1 receptors to create anxiolytic effects, sedation, nerve calming, muscle relaxation, and postural instability. Some classes of anxiolytics, including benzodiazepines such as Diazepam, are mostly used for their amnestic effects and antiepileptic action. In the case of over-stimulation postsynaptic
Compare and Contrast Different Treatment Options for GAD
SSRIs, SNRIs, and Benzodiazepines are effective in managing GAD. A common SSRI, Sertraline (Zoloft), is a first-line treatment option for GAD due to its positive biological and psychological effects (Lux & Harry, 2021). Based on the evidence, Diazepam, a Benzodiazepine, has a higher efficacy than Imipramine, TCA. In a comparative efficacy study, benzodiazepines were noted to have better efficacy than antidepressants, making it more comparable to the action of first-line drug options belonging to the SSRIs and SNRIs classes (Du et al., 2021). Regardless, the choice of medication for GAD is based on the specific pathophysiology of the illness.

Question
Generalized Anxiety Disorder is a psychological condition that affects 6.1 million Americans, or 3.1% of the US Population. Despite several treatment options, only 43.2% of those suffering from GAD receive treatment. This week, you will review several different classes of medication used in the treatment of Generalized Anxiety Disorder. You will examine the potential impacts of pharmacotherapeutics used in the treatment of GAD. Please focus your assignment on FDA-approved indications when referring to different medication classes used in the treatment of GAD.

Treatment of Generalized Anxiety Disorder
Reflect on your experiences, observations, and/or clinical practices from the last 5 years and think about how pharmacokinetic and pharmacodynamic factors altered his or her anticipated response to a drug.
Consider factors that might have influenced the patient’s pharmacokinetic and pharmacodynamic processes, such as genetics (including pharmacogenetics), gender, ethnicity, age, behavior, and/or possible pathophysiological changes due to disease.
Think about a personalized plan of care based on these influencing factors and the patient history with GAD.
Post a discussion of pharmacokinetics and pharmacodynamics related to anxiolytic medications used to treat GAD. In your discussion, utilize the discussion highlights to compare and contrast different treatment options that can be used.
