Medication error is a lack of correct treatment for a patient and may cause harm. Medication errors may occur intentionally or unintentionally. As the nursing manager, I would first do an investigation to know if the medication error made by a nurse two days in a row was either a system problem or a problem related to the individual nurse (Mehrabifar et al., 2017). To identify the medication error, I would first do a chart review. A chart review contains recorded data and can help to identify medication errors since it contains information based on laboratory data, prescription data, and administrative data. Secondly, I would follow up with the patient and see if they were given the correct prescription according to their ailment. The prescription and drugs issued should be the correct medication for the patient’s disease. Finally, I would check the system’s functionality to see whether the system may be involved in the medication error (Härkänen et al., 2015). I would check with the person controlling the systems and ask them to run a quick check-up to detect if there is a fault in the systems. These methods would help detect whether the medication error was individual or system-based.

To correct the medication error, I would ensure that my department follows the proper medication reconciliation procedures by reviewing and verifying that a patient receives the correct medication, dosage, transfer orders, and the correct medication listed in the transfer documents. I will also double-check procedures by ensuring that each patient’s order is correctly transcribed, administered, and treated through the administration records (Rodziewicz & Hipskind, 2018). I would also demand that there be documentation and labeling of every medication. Finally, I would ensure that every nurse/surgical unit has access to a drug guide available at all times.
