Studies show that health portals, which are one type of nursing informatics, could meet the growing demand for healthcare services and help with nursing practice. Health portals help educate patients with a large body of health information, improve self-care skills and medical decision-making, and help the patient maintain good physical, psychological, and social well-being (Tao et al.,2018). Patient portals are secure online websites that are part of the electronic health record and are the only way patients can access their health information and health professional notes. By accessing their health information, patients feel more engaged in managing their health condition and appreciate a better way to interact and collaborate with their healthcare team. The stakeholders who will impact by this project include patients and their families, clinical leader, administrator, and nurse informatics. Nursing informatics (NI) combines nursing science with information and analysis. It defines, manages, and communicates data, information, Knowledge, and wisdom in nursing practice. To achieve desired outcomes, NI support nurses, consumer, patients, the healthcare team, and other stakeholders in their decision–making (Sipes,2016).

Nursing informatics provides a mechanism for patients to provide their clinical information and to share it with family. This information enables the patient to be in control of their health. Mobile health apps, combined with telehealth and remote patient monitoring approaches, are effective in the management of many diseases and are designed to reduce hospital readmissions (NG et al.,2018). The patient portal is an efficient way of communication that improves the quality of care. Nurse informatics plays a role in the design and implantation of strategies for health care applications.
