The program includes a mobile app and a wearable bracelet that will monitor blood pressure, heartbeats, respiratory rate, and other heart-related parameters. The app provides access to related health information, including strategies to maintain a healthy heart and weight management support that provides guidance on exercises and physical activity, diet, and reminders for taking medications and checkups. The Innovations that the Service Will Bring to the Industry

The mobile app and bracelet are a part of the application of mHealth in delivering healthcare services that are either curative, maintenance, or preventative services. Mobile health (mHealth), which includes wearables and applications, offers new ways to communicate the health information people need for self-care and maintenance of their health. In addition to offering the chance to continuously monitor heart health, mHealth apps have been shown to be successful in promoting and maintaining preventative behavior changes, offering adequate digital support for self-management of a particular condition, and offering direction for self- and remote diagnosis (Rowland et al., 2020).

The heart disease management and education programs using mobile apps and the bracelet are innovatively changing how health education on heart disease includes risk factors, how to manage current conditions, and how the advancement or the development of the condition can be managed. The mobile app also has an interactive chat function that the users can directly contact health providers, ask questions, and gain insights on a specific issue of concern. This changes how the community maintains heart health and prevents heart disease and related complications. The use of the app also provides an opportunity to collect and analyze big data to identify trends and risk factors specific to the local Sawyerville community and at the Hale County level.

A notable part of the mobile app is that it supports integration with the current electronic health records (EHRs) systems and telemedicine systems. EHRs and telemedicine are among the most important digital health technologies today. EHRs help overcome collaboration challenges in remote settings to improve service delivery (Dobrow et al., 2019), while telemedicine has proved to provide remote health management necessary for continuity of care in outpatient settings (Sun et al., 2021). Telemedicine will be used to support remote monitoring and self-care, while the patient will have the option to link their app to the patient portal to continuously update their health records.
