Proposed Project Interventions
The Extent of Evidence-Based Data for Proposed Interventions
My proposed interventions address three themes. They include venous thromboembolism (VTE) prophylaxis, ultrasound screening, and risk stratification. VTE prophylaxis embraces both medications and non-pharmacological approaches to offer a protective effect against deep vein thrombosis. Examples of pharmacological interventions include unfractionated heparin, fondaparinux, and low molecular weight heparin, whereas compression stockings are an example of non-pharmacological interventions (Gao et al., 2022). Ultrasound screening plays a significant role in detecting suspected cases of VTE (Galien et al., 2021). This forms the basis of optimizing pharmacological approaches to obtain optimal outcomes. Li et al. (2022) report that risk stratification enables healthcare providers to make informed decisions and adopt individualized care for patients.
Factors that Influence the Use of Proposed Interventions and Potential Barriers
Various factors will influence the use of the proposed interventions. Firstly, interdisciplinary collaboration will promote the success of the interventions (McArthur et al., 2021). Collaboration among interdisciplinary team members will promote care coordination and patient monitoring and promote the success of the interventions. Secondly, adequate resource allocation will promote the success of the intervention (McArthur et al., 2021). For instance, resources will facilitate training members of the interdisciplinary team on VTE prophylaxis, ultrasound screening, and risk stratification.
Potential barriers include limited knowledge and skills and limited human resources. Members of the interdisciplinary team may be unaware of the current VTE prophylaxis protocols. Furthermore, they are likely to be unaware of the right administration techniques. This increases the risk of medical errors. This barrier will be addressed by comprehensive training and education. The intervention requires collaboration and care coordination. However, the ICU may lack enough staff to monitor patients continuously. This may impede the success of the interventions.
Resources
Human resources will be required to accomplish care coordination in the ICU unit. Notably, the ratio of nurses to patients should be 1:2. Human resources will avert shortages and reduce incidences of burnout and medical errors associated with shortages (Winter et al., 2020). Human and monetary resources will be required to facilitate the training and education of members of the interdisciplinary team.
Implementation Activities and Timeline
The implementation will begin a budget and roadmap will be created within one week. Thereafter, both human and monetary resources will be allocated based on the project’s budget. Resource allocation will take place within three weeks. Next, interdisciplinary team members will be trained on VTE prophylaxis, ultrasound careening, and risk stratification. The training session will occur within one month. Refresher training will be done monthly after the implementation of the intervention. Lastly, the project will be implemented based on the roadmap.
Monitoring, Tracking, and Ongoing Review
Patient’s medical records will facilitate monitoring, tracking, and review. This will be accomplished by a quality assurance team comprised of nurse practitioners, physicians, and pharmacists. Monitoring and tracking will begin from patient admission to the ICU and end upon discharge or referral. This exercise will ensure that all interventions have been implemented as planned. Furthermore, monitoring and evaluation will form the basis for review in the event of adverse effects or suboptimal outcomes.
Performance of Tasks Required for Implementation
Qualified nurses, physicians, pharmacists, and laboratory technicians are critical to the implementation of the intervention. These healthcare providers will be involved in patient monitoring. Nurses will promote safe medication administration during VTE prophylaxis. Physicians will prescribe evidence-based medications, whereas pharmacists will provide information about the rational use of the medications. Laboratory technicians will play a key role in ultrasound screening.
Strategies that Facilitate the Implementation of the Proposed Intervention
As earlier stated, training and education will ensure that members of the interdisciplinary team demonstrate pertinent competency when implementing the interventions. Also, adequate resource allocation will facilitate the implementation of the intervention. This will be used to avert shortages and train all stakeholders. Thirdly, an emphasis on care coordination and collaboration will facilitate the implementation of the interventions.
