Care to Delegate to the LPN and CNA

A CNA and LPN play vital roles within the hospital setting. In the scenarios assigned to the LPN, he has several assessments that he should undertake and report back to me as the registered nurse. For Client Number 1, with a recent heart attack, the LPN should perform a comprehensive cardiovascular and respiratory examination. Any findings should be reported to me to facilitate the further assessment of the patient.

Interventions to Supervise

As the RN in charge, it is essential to supervise both the LPN and the CNA. As the RN, it is essential to supervise the LPN while undertaking both the cardiovascular and respiratory examination for Client Number 1 and ensuring that all findings are reported to me. I should also supervise the administration of oral medication that is to be undertaken by the LPN for all the assigned patients. Formulating a medication plan and going through it with the LPN is a critical aspect. I should keep track of the daily vital signs recorded and note any sudden changes. Supervising tasks the LPN and CNA undertake for the first time is also essential.

Priority of Care

The 18-year-old male (Client number 4), wheezing and has labored respirations unrelieved by an inhaler, should be the first person to attend to. The patient’s current situation is critical and highlights the need to undertake additional interventions to prevent progressive respiratory depression. The patient with a fractured femur (Client number 3) should be prioritized second. The patient is in obvious pain, and pain management, including medication and repositioning, should be prioritized (Cui et al., 2018).

The patient with full-thickness burns (Client number 2) that requires dressing should come third. The patient requires regular changing of dressing that requires collaboration between nurses. Medication should be administered to ensure that the patient does not experience discomfort. The dementia patient (Client number 5) should be prioritized next. Caution should be taken to ensure the patient does not hurt himself, and effort should be taken to reorient him. Laboratory results should be monitored to note any infections causing the dementia.

Client number 1 is in stable condition and should be prioritized fifth. Constant monitoring of his vital signs, heart sounds, and pain monitoring should continue. Client number 6 comes last. She is currently being discharged. Follow-up on her condition to ensure compliance with medication and other recommendations should be undertaken.
