Preparing for My QI Project and Clinical Practicum- Initial Steps and Planning
I am A_N. I am starting my 30 hours of practicum. My practicum will take place in the primary care setting. I have selected A-N_O, an MSN-prepared advanced practice nurse, as my preceptor throughout my practicum. She has been working as a registered nurse (RN) for twelve years and as an APN for 5 years, giving her 17 years of extensive experience as a nurse. Working in the primary care settings, I will majorly focus on providing care to pediatric patients. The clinical concern of interest that I have identified within the pediatric clinical setting is juvenile diabetes. Juvenile diabetes, also known as Type 1 diabetes, is an auto-immune-mediated that develops due to the destruction of insulin-producing B-cells in the pancreas (Eizirik et al., 2020). Of concern is the alarming rate of the increase in the incidences of juvenile diabetes at a rate of 3-4% in the last few years, as well as the associated complications and risk of developing Type 2 diabetes (Norris et al., 2020).
I will work collaboratively with my preceptor to search for research evidence to help understand juvenile diabetes and inform evidence-based practices when managing pediatric patients with juvenile diabetes. I will also consider and compare various evidence-based interventions for the effective management of juvenile diabetes within pediatric care settings. Additionally, I will also work closely with my preceptor to develop a case study on juvenile diabetes as well as create a personalized patient care plan, including patient and family education and addressing any concerns with available pharmacological and non-pharmacological care strategies to be employed in the case study. The overall goal of doing my practicum hours in the primary care setting caring for pediatric patients with juvenile diabetes is to improve my clinical experience and knowledge to help improve juvenile diabetic patients’ outcomes, improve patient safety, and the overall quality of care for this particular patient population.

This week, you will review your Practicum Guide, review how to use the DCN databases, upload your preceptor paperwork, brainstorm with your peers what your QI project will be and start scheduling your 30 hours of practicum or clinical experience.
This week, you will identify a problem or clinical concern of interest with your selected population, obtain an MSN preceptor with a minimum of one year experience as an RN, upload necessary paperwork, complete any mCE requirements, and identify your clinical setting.
