Your Practicum experience includes working in a clinical setting that will help you gain the knowledge and skills needed as an advanced practice nurse. In your practicum experience, you will develop a practicum plan that sets forth objectives to frame and guide your practicum experience: Practicum Experience Plan.
As part of your Practicum Experience Plan, you will not only plan for your learning in your practicum experience but also work through various patient visits with focused notes as well as one (1) journal entry.

Part 2: Individualized Practicum Learning Objectives
Refer to the instructions in Week 2 to create individualized practicum learning objectives that meet the requirements for this course. These objectives should be aligned specifically to your Practicum experience. Your objectives should address your self-assessment of the skills found in the “PMHNP Clinical Skills Self-Assessment Form” you completed in Week 1.
As you develop your individualized practicum learning objective, be sure to write them using the SMART format. Use the resources found in Week 2 to guide your development. Once you review your resources, continue and complete the following.
Note: Please make sure each of your objectives are connected to your self-assessment. Also, consider that you will need to demonstrate how you are advancing your knowledge in the clinical specialty.
Objective 1: During this clinical rotation, I plan to strengthen my diagnostic reasoning by formulating accurate differential diagnoses and prioritizing conditions for at least five patients weekly with preceptor guidance. I aim to refine my ability to distinguish pathophysiological from psychopathological symptoms using DSM-5-TR criteria.
Planned Activities: I will assess patients independently and formulate diagnostic impressions. I will engage in weekly supervision to discuss differential diagnoses and incorporate feedback to enhance accuracy.
Mode of Assessment: Each week, my preceptor will evaluate five of my diagnostic plans and provide written/verbal feedback in Meditrek (Burgess et al., 2020).

- My professional plan is to develop the clinical judgment required to formulate appropriate psychiatric diagnoses across the lifespan, aligned with advanced practice standards.
Objective 2: By mid-practicum, I will independently select and administer at least four validated screening tools (e.g., PHQ-9, GAD-7, MMSE, CAGE) tailored to patient age, diagnosis, and clinical need. I will interpret and integrate results into care plans, treatment rationale, and documentation. This goal will improve my skill in applying objective assessment data to real clinical contexts and enhance accuracy in diagnosis and planning.
I will discuss my choice and findings with my preceptor during weekly supervision. Using screening tools helps detect hidden symptoms and guides referrals and treatment decisions. These skills are vital for quality psychiatric-mental health care.
Planned Activities: Identify patients needing screening; administer tools; document and discuss with preceptor.
Mode of Assessment: Evaluation of tool use and interpretation via documentation and Meditrek.

Objective 3: During this practicum, I will develop my ability to engage in evidence-based psychotherapy by providing cognitive behavioral therapy (CBT) to at least six clients with different age groups. My goal is to use systematic CBT procedures to treat disorders like depression, anxiety, and post-traumatic stress disorders. I will emphasize developing individualized treatment plans that consider each client’s stage of development, cultural orientation, and presenting problem. I will reassess these plans bi-weekly based on observed therapeutic progress, supervisor and client feedback, and the goal of enabling measurable improvement in mental health outcomes.
Planned Activities: I will conduct initial intake and ongoing therapy sessions with CBT strategies such as cognitive restructuring, behavioral activation, and guided journaling. I will create a case conceptualization for each client and establish target behaviors or thought patterns to address (Chand et al., 2023). I will discuss the effectiveness of each intervention bi-weekly and examine potential changes with my preceptor. In addition, I will maintain intense session notes and read current literature to support my practice.
Assessment Mode: Evaluation will be direct observation by my preceptor, review of clinical documentation in patient charts, Meditrek entries, and verbal feedback through weekly supervision. I will also provide reflective summaries of progress and challenges in therapy.

- Apply psychotherapeutic modalities that are developmentally appropriate, culturally sensitive, and grounded in current evidence-based practice to support recovery and symptom reduction in clients receiving psychiatric care (Nakao et al., 2021).
Part 3: Projected Timeline/Schedule
Estimate how many hours you expect to work on your Practicum each week. *Note: All of your hours and activities must be supervised by your Preceptor and completed onsite. Your Preceptor will approve all hours, but your activities will be approved by both your Preceptor and Instructor. Any changes to this plan must be approved.
This timeline is intended as a planning tool; your actual schedule may differ from the projections you are making now.
I intend to complete the 144 or 160 Practicum hours (as applicable) according to the following timeline/schedule. I also understand that I must see at least 80 patients during my practicum experience. I understand that I may not complete my practicum hours sooner than 8 weeks. I understand I may not be in the practicum setting longer than 8 hours per day unless pre-approved by my faculty.

- Select one nursing theory and one counseling/psychotherapy theory to best guide your clinical practice. Explain why you selected these theories. Support your approach with evidence-based literature.
- Create a timeline of practicum activities that demonstrates how you plan to meet these goals and objectives based on your practicum requirements.
The Assignment
