Medication errors significantly impact patient safety. Therefore, the objective of this project is to improve patient safety by determining and reducing the major causes of medication errors and applying tailored preventive strategies. To achieve this goal, the FADE model will be used to ensure that the implemented interventions are effective and evidence-based. A systematic approach is necessary to decrease an organization’s susceptibility to errors by providing the required resources to monitor, analyze and implement effective interventions (Elden & Ismail, 2016).

The intervention implementation plan will start off with the intervention requiring the least training and resource aids. This intervention is simply comprised of a two-nurse verification sign-off whenever a nurse transcribes a physician’s order into the eMAR. Since there are always two nurses working side-by-side running the unit, having a second nurse verify that the order was transcribed correctly is straightforward ad cost-effective. Chart checks have been proven to be an essential piece to increasing safety and efficiency in a department and reducing unforeseen events during a patient’s treatment (Lewis & Heath, 2018).

A more efficient self-reporting process is an intervention that would prove beneficial in monitoring errors and identifying trends. The Joint Commission on Accreditation of Health Care Organization reemphasized the importance of the analysis of error reports to prevent future errors with the implementation of additional patient safety standards that address the development of a culture of safety (Joint Commission Accreditation of Health Care Organization (JCAHO), 2017). Collected data from the Quality Improvement Officer show a questionably low number of officially reported med errors, and of these errors, less than half are self-reported. During the implementation process, nurses will be made aware of the gaps found in the current process and how those gaps contribute to the increased number of medication errors, thereby impacting the safety of the patients. The staff will be trained on the new process and educated on the new process and how it will mitigate future errors by providing more accurate tracking. Error reporting helps to understand why errors occur, prioritize opportunities for error prevention and generate long-term improvements in patient safety (Claudia et al., 2016). Incident reporting forms will be made more accessible both electronically and in hard copy. Voluntary reporting by healthcare providers depends on their awareness; hence many errors remain frequently unreported (WHO, 2014). Nurses will be made aware of where to locate these forms and the types of incidents that warrant reporting. Adoption and proper use of self-reporting will make it easier to identify and mitigate error-causing factors.
