The target population in this assignment is the residents of Vinings Lake SW, Mableton, GA. This community is served by this organization and the main constituents are families, young adults, and elderly individuals. The organization focuses on providing support and resources to improve health outcomes, particularly for those facing economic and social challenges.
Social Determinants of Health (SDOHs):

The demographic of this community is affected by the following actual and potential social determinants of health as follows.
1. Economic stability is affected by the unemployment and income levels. For instance, the unemployment rates have resulted in financial instability which in turn has made it difficult for the members of this community to afford basic necessities. Also, the low-income levels in the community have compromised the ability to meet the cost of quality healthcare, essential resources, and nutritious food. Lastly, the financial constraints may affect the mental health of the members of this community.
2. Education levels affect the level of health literacy where limited education can lead to lower health literacy where the residents may not understand the health information and the various ways to navigate through the healthcare system. Also, limited education can compromise the ability to secure employment opportunities for the young people of this community which may affect their access to quality health services.
3. Healthcare access and quality is another determinant of health observed in this community where the shortage of healthcare providers and facilities may fail to meet the health needs of this community leading to untreated conditions and poor health outcomes. Lack of health insurance for significant members of this community may make it difficult to afford necessary medical treatments.
4. The neighborhood and physical environment are other determinants of health that impact the demographics of this community. To begin with housing conditions such as safety, overcrowding, and maintenance may positively or negatively influence mental and physical health issues. Secondly, transportation services can hinder access to healthcare and other vital services such as schools and areas to seek employment. Lastly, environmental hazards can compromise the health of the residents.
5. Social and community context is the other social determinant of health of this community that influences the demographics of this community through social isolation and community support system. For instance, isolation of the vulnerable in the community such as the elderly and the mentally ill may worsen their health status as well as compromise the access to healthcare services (Chelak & Chakole, 2023).

Rationale of Topic:
• Explain the reason for your choice of education topic based on your research.
Based on the community assessment, avoidable health challenges emanating from different causes such as poor nutrition and unhealthy eating behaviors are prevalent in this community. Some of these conditions include obesity, diabetes, and heart disease. This finding rationalizes the selection of my topic and tailors me towards attaining my objective to conduct educative sessions on the primary prevention measures to avoid the occurring of the aforementioned conditions as well as promoting health for the cases that are already there to avoid complications and improve the quality of life.

Throughout this course, students will be working on the Community Teaching Project. The project will include a community teaching presentation given during Topic 5. The audience for the presentation will be community members, rather than the staff at the community agency chosen or in-patient residents.

NRS-425 Selection, Research, and Assessment
The Community Teaching Project has indirect care experience requirements. The “Indirect Care Experience Hours” form, located in Topic 1 resources, will be used to document the indirect care experience hours completed in the Community Teaching Project and presentation. As progress is made on the Community Teaching Project, update this form indicating the date(s) each section is completed. This form will be submitted in Topic 5.
In this topic, students will be selecting, researching, and assessing a nonprofit community setting in their community using one of the following four topics:
- Primary prevention/health promotion
- Secondary prevention/screening for a vulnerable population
- Disaster planning
- Environmental issues
The following are considered appropriate nonprofit community organization settings:
- Public health clinic
- Community health center
- University/school health center
- Religious community (i.e., local church)
- Community center (i.e., YMCA)
- Adult/childcare center
- Youth development organization (i.e., 4-H, scout troops, Boys and Girls Clubs, etc.)
- Senior centers
- Food pantries (St. Mary’s Food bank, St. Vincent De Paul, etc.)
