NR 439 Week 2 Discussion – Search for Literature and Levels of Evidence

Having worked in the healthcare field for several years, I have noticed several clinical issues and practices that can be changed for the benefit of the patients, the nurses, the physicians, and all the other stakeholders in healthcare. I have worked with the Emergency Department in the recent past, and most of the patients that I have dealt with are those who are in pain and require immediate addressing. In most cases, when the patient’s issue has been addressed, the patient becomes more comfortable, and it is then easy for the nurse or physician to talk to the patient and address the other issues (Houser, 2018). Therefore, I have come to the conclusion that it is important to address the pain needs of the patient immediately so that the nurse or physician can have a good time diagnosing the patient appropriately and ensure positive patient outcomes. Pain is always a top concern for patients, and when it is relieved quickly, the reviews for the emergency department and the concerned healthcare professional will increase. On the other hand, when a patient believes that his/her pain is not being addressed quickly, the same will also be reflected in the reviews and patient satisfaction scores.

This topic is important to me because I consider it as the first step in a proper diagnosis of the patient and in ensuring positive patient outcomes. I believe that in the Emergency Department when a painting patient is made comfortable by addressing the pain needs immediately and quickly, the nurse and the physicians then get time to talk to the patient leading to a proper diagnosis (Dewhirst et al., 2017). This is as opposed to treating a patient who is in pain, is stressed, and does not think that the nurses and physicians are doing enough to make him/her comfortable. Therefore, I chose this problem/topic because it is the first and most important step for nurses and physicians in the emergency unit.

Clinical question – PICO (T):

Among adults in the Emergency Department that are in pain, would a registered nurse-initiated STAT analgesic orders be more effective than waiting for physician analgesic orders in reducing pain among patients and consequently improving the patient satisfaction scores in less than 2 hours?

P = Adults in the Emergency Department that are experiencing pain. I = registered nurse-initiated STAT analgesic orders.

C = waiting for physician analgesic orders

O = reducing pain among patients and consequently improving patient satisfaction scores. T = 2 hours.

I care so much about the pain needs of the patient as a nursing practice problem because it is an initial step to achieving the best patient outcomes. Patients who are in pain, especially in the emergency department, will definitely judge the nurse or the physician based on their ability to address the pain needs first before everything else. I believe that this problem would benefit from finding the best evidence because nurses and other healthcare professionals will not only see a reason to first address the pain needs of the patient but will also learn the best approach to addressing this need when it occurs.

NR 439 Week 2 Discussion – Search for Literature and Levels of Evidence

Reflect on your practice, and identify a significant nursing clinical issue or change project that you would like to search for evidence in online sources. Formulate searchable clinical questions in the PICO(T) format for your nursing clinical issue.

Next, review the guidelines for the PICO(T) Assignment due week 3. Use your PICO(T) elements to search for a report of a single, original study that has been published within the last 5 years from the CCN Library that is relevant to your nursing clinical issue.

Briefly describe how it is relevant to your nursing clinical issue. Remember to give complete reference to the study.
