Instructions:
Use this worksheet to gather relevant information that you will use to complete the “Affiliation Recommendation” in Task 2. This worksheet derives from the critical factors for affiliation discussed in step 1 of the Strategic Partnering and Affiliation Guide (adapted from Grube and Finnerty’s “How to Make the Strategic Partnering Decision”) and has been modified to fit the assignments. This learning resource is located in the course: Framework for Affiliation Analysis Worksheet (Assignment 1).
For this assignment, use the “Twin Pines Medical Center Profile” provided in the course to find the necessary information. Be aware that to answer the questions in the following table, you will need to draw inferences or make reasonable assumptions about Twin Pines Medical Center (TPMC).

What are the community’s demographic and market needs? Based on those needs, what should be TPMC’s goals for affiliation? |
Such a community served by TPMC is plagued with 33% high rates of being uninsured and the incidence rate for diabetes being very high at 17.9%, with disproportionately excessive burdens among non-Hispanic Black and Native American populations. These are typically underserved segments that continue to seek the ED for advanced care in diabetic conditions for want of preventive services, leading to longer waiting times and an increase in overall treatment costs.
Some affiliation goals for TPMC include increasing specialty care access, offering telemedicine interventions to support earlier interventions for diabetic patients, and improving health promotion and prevention educational programs (Anderson et al., 2022). The result will be reduced ED utilization, better health outcomes, and the fulfillment of needs among those vulnerable populations currently served by TPMC. |

Some of the ongoing initiatives by the company include the development of a “culture of quality,” creating employee engagement to improve the experiences of patients, and practicing cost-effective methods.
Critical strategies for meeting the community’s needs include integrating specialty care for chronic conditions such as diabetes and expanding the organization’s telemedicine capabilities to reach more vulnerable populations. TPMC could move to accept technological solutions provided by the partners, extend employee training, and share resources to attain the goal of efficiency and effectiveness in care delivery (Beer & Mulder, 2020). |

What clinical services does TPMC provide? In what areas can TPMC improve the quality of the services provided? What are the goals for improving clinical services and quality? What are the desired healthcare outcomes of an affiliation? |
These include family medicine, women’s health, behavioral health, dental care, optometry, and emergency care, among other clinical services offered at TPMC.
However, when it comes to specialty services like endocrinology, diabetes education, and nutritionists, which are very important for helping deal with this rising burden of diabetes, services at TPMC are scant.
Some improvement goals identified for clinical service include enhancement of chronic disease management, preventive care, and educational outreach to empower patients (Stepanian et al., 2023).
Desirable healthcare outcomes associated with an affiliation include decreased ED utilization, better management of the complications of diabetes, improved patient quality of life, and reduced total healthcare costs. |

The clinical expertise at TPMC includes family medicine alongside internal medicine and behavioral health, together with women’s health services.
To fulfill its goals, TPMC needs to add specialist medical staff, such as endocrinologists, alongside certified diabetes educators and nutritionists (Ganetsky et al., 2020). Further, existing staff must undergo training that focuses on chronic disease management while learning preventive care strategies. The affiliation will supply essential clinical experience, enabling TPMC to offer a wider range of medical services to the population. |

What facilities and technical support are available to TPMC? What facilities and technical support will TPMC need to meet the goals of an affiliation? |
To serve its patients TPMC runs a 34-bed hospital and outpatient clinics while offering pharmacy services and radiology facilities along with dental services. The current facilities fail to provide required technical resources necessary for specialized diabetes care along with telemedicine services.
The successful fulfillment of affiliation goals at TPMC requires funding to establish a telemedicine consultation system that includes complex diagnostic instrumentation and enhanced information technology infrastructure for care team cooperation (Haleem et al., 2021). With a partnership with a larger organization, those are just some of the resources TPMC would gain access to and, in return, be in a position to give its community cost-effective, quality care. |
