Discussion – Patient-Centered Care

I work at the Acuity Adaptable (AA) unit at Eskenazi Hospital. Working in an AA unit means providing all care services in the same unit without having to transfer the patient to another unit. I worked with Mr. KT, a patient who had been diagnosed with diabetes and comorbid hypertension. He was also overweight and was approaching obesity levels. I realized the patient was concerned about his health as he did not follow the care plan as advised. After making this discovery about the patient’s lack of involvement in his care, the patient was advised and encouraged to be more involved in his own care. Notably, patient participation involves taking an active role in one’s care, treatment, and support (McCarron et al., 2019). The advice included engaging in regular physical activity, weight reduction, and active engagement in making decisions relating to the care delivered, including medications.

Taking an active role in his care and treatment, KT has had notable changes and improvements in his health. Firstly, he now best understands his condition as well as other comorbid conditions. Secondly, Mr. KT has also been able to adhere to his medications while, at the same time, taking the right medication and reducing the risk of medication errors. In addition, the involvement of the patient in his health has had notable changes in Mr. KT’s body weight. Noteworthy, Eskenazi Hospital has a patient quality and safety culture that supports the provision of patient-centered care to all patients. Combining patient-centered care with improved patient participation in their health can help patients like Mr. KT improve their health outcomes, as well as the patient’s experiences during the delivery of the care.
