Client Information

James is a 38-year-old African American cisgender heterosexual male. He is divorced. James is best described by the intracategorical multiplicity of demographic identity. He is a religious man who subscribes to the traditional African faith, which he says is hard to come by. He is a first-generation immigrant, having immigrated to the U.S. ten years ago. He is a graduate software engineer working for a firm in Silicon Valley. He is well-off and can meet most of his daily needs, but he lives in a rental apartment. James has a significant history of alcohol abuse and cigarette smoking. He had been suspended from work on two separate occasions for presenting to work drunk and in a poor mental state. He was raised by a two-parent heterosexual couple, both of whom are still alive. There is no history of substance abuse in James’ nuclear family. Presenting Concerns and Goals 

James was referred to the clinic for the management of major depressive disorder with substance abuse disorder. He has been presenting symptoms of hopelessness, a persistent low mood, diminished interest, feelings of worthlessness, fatigue, and loss of concentration for over a year now. He has also had suicidal ideation and attempts twice in the past six months. These manifestations are consistent with those of major depressive disorder described in the DSM (Christensen et al., 2020). Suicidal tendencies manifested by the client are particularly problematic as they present a threat to the self. Dendup et al. (2020) report that suicidal tendencies present some urgency to mental health care due to the potential harm they have on clients with mental health illnesses. During the assessment, his neighbor and friend who accompanied him to the clinic noted that these manifestations began after he divorced his wife and that he has been avoiding counseling because he believes that his predicaments result from a curse from his late grandfather. The treatment goal for this client is to lessen depressive episodes using pharmacological and non-pharmacological interventions and to help the patient quit substance abuse.

Client and Helper Values

The client is spiritual and believes in curses. From his neighbor’s account, he links his suffering to a curse from his grandfather. The self-fulfilling prophesy effect of curse beliefs may not only undermine an individual’s confidence in their future success but also elicit the perception of unavoidable misfortunes (Ghrear et al., 2021). The helper’s values are not aligned with the client’s. The helper is an existentialist, believing in the scientific explanation of the disease. The helper refutes the client’s claim that his suffering results from an inevitable misfortune and advocates for sound management to lessen the client’s suffering. An ethical concern that may be apparent in this case is informed consent. The client may fail to give informed consent to facilitate his management.

Social Justice Competencies

Several inequities are apparent in contemporary society. Gaps in healthcare still exist, with many caregivers having less appreciation of differences in religious worldviews and their influences on health (Ferreira et al., 2022). The client in the case has encountered this inequity in his pursuit of wellness. Social inequities may impede the client’s health-seeking behavior. Educating the client on differences in religious beliefs and differences in worldviews may enhance his healthcare-seeking behavior.

To prevent these biases, the counselor can make considerate decisions, avoid assumptions about the client, and learn more about the individual or the group. Notably, cultural humility fosters a better understanding of different cultures and clients’ worldviews; thus, it is necessary for healthcare. Interacting with cross-cultural clients helps widen the counselor’s worldview and culture and is thus important in healthcare.

Minimally, include language to show how you would acknowledge the client’s concerns and present your treatment or intervention recommendations.
