Clinical Skills-Applying for an NPI

The National Provider Identifier (NPI) is a 10-digit number that the government issues to healthcare providers as a unique identifier for each healthcare provider. The NPI number is a legal requirement for all covered providers under the Health Insurance Portability and Accountability Act (HIPAA). It provides information on payers and other partners under a specific provider (Centers for Medicare & Medicaid Services (CMS), 2021). All covered healthcare providers and their partners must use the NPI when making Medicare insurance claims. The NPI is also required for one to be able to enroll and disenroll from CMS services, to coordinate and facilitate payment of benefits and premiums, for remits, referrals, and payment authorizations. The NPI can be applied in three ways: through the online portal provided by CMS, through a paper mail application, or through an authorized CMS contractor. Notably, the online application is the most recommended due to its time efficiency in the application process. To make an online application, one must first visit the National Plan and Provider Enumeration System (NPPES) website. The person or group seeking to make the application will be required to create a username and password via the Identity & Access Management (I&A) System (Arias, 2021). The created username and password are used for login purposes.

Further, for the mail application, one must request or download an application form for the NPI Enumerator, fill in all the required fields, sign, and ensure the filled form is delivered to 7125 Ambassador Road, Ste 100 Windsor Mill, MD 21244. Lastly, applications via CMS contractor will require the applicant to allow an Electronic File Interchange Organization (EFIO) to access their data and send it through a bulk enumeration process (Arias, 2021).

You must have an NPI number for practice as a nurse practitioner through the National Plan & Provider Enumeration System. An NPI is a 10-digit numerical identifier for health service providers. It is national in scope and unique to the provider. Discuss how you can apply for an NPI.
The National Plan & Provider Enumeration System issues the NPI, a Health Insurance Portability and Accountability Act (HIPAA) Administrative Standard. An NPI is a unique identification number for covered healthcare providers, created to improve the efficiency and effectiveness of the electronic transmission of health information (Centers for Medicare and Medicaid Services, 2019). Covered healthcare providers, all health plans, and healthcare clearinghouses must use NPIs in their administrative and financial transactions (Centers for
Medicare and Medicaid Services, 2019).
First, the applicant must accept the warning about accessing a U.S. Government information system and consent to have “no reasonable expectation of privacy regarding any communication or data transiting or stored on this information system. The government may monitor, intercept, search, and seize any communication or data transmitted or stored on this information system. If the applicant declines, they cannot continue (National Plan & Provider Enumeration System, 2020).
Secondly, the applicant must gain an understanding of the 2 entity types:
Entity Type 1: “Individual health care providers may get NPIs as Entity Type 1. As a sole proprietor, one must apply for the NPI using their own SSN, not an Employer Identification Number (EIN) even if they have an EIN. As a sole proprietor, one may get only 1 NPI, just like any other individual. For example, if a physician is a sole proprietor, the physician may get only 1 NPI (the individual’s NPI)” (National Plan & Provider Enumeration System, 2020).
“An incorporated individual is a single health care provider who forms and conducts business under a corporation. A sole proprietor isn’t an incorporated individual because the sole proprietor didn’t form a corporation. If you’re a sole practitioner or solo practitioner, it doesn’t necessarily mean you’re a sole proprietor and vice versa” (National Plan & Provider Enumeration System, 2020).
“Entity Type 2: Organization health care providers are group health care providers eligible for NPIs as Entity Type 2. They may have a single employee or thousands of employees” (National Plan & Provider Enumeration System, 2020).
“The NPI Final Rule refers to the components and locations as subparts. An organization’s health care provider can get its subparts their NPIs. If a subpart conducts any HIPAA standard transactions on its own (separately from its parent), it must get its own NPI” (National Plan & Provider Enumeration System, 2020).
“Subpart determination ensures that entities within a covered organization are uniquely identified
in HIPAA standard transactions they conduct with Medicare and other covered entities. For example, a hospital offers acute care, laboratory, pharmacy, and rehabilitation services. Each subpart may require its own NPI because each sends its standard transactions to 1 or more health plans” (National Plan & Provider Enumeration System, 2020).
Suppose you’re an individual healthcare provider who’s incorporated. In that case, you may need to get an NPI for yourself (Entity Type 1) and an NPI for your corporation or LLC (Entity Type 2) (National Plan & Provider Enumeration System, 2020).
