Care coordination is a fundamental aspect of mental healthcare management. Care coordination involves the deliberate organization of healthcare professionals to achieve effective and safe care. The benefits of coordinated care are evident both in the in-patient hospital care and in the care continuum at home and in institutionalized care. The suitability of this care approach in chronicity underpins its utility in mental health disorders. Of importance in care coordination is that every healthcare professional involved understands their role and can work in concert with others toward the design of effective therapeutic plans. In this presentation, we focus on care coordination during the management of mental health illnesses.

Effective Strategies for Healthcare Collaboration

Healthcare collaboration is the gold standard for chronic care. Collaborations in healthcare may be between the individual healthcare professionals present in healthcare teams or between healthcare providers and their patients and patients’ families. Healthcare collaborations aim to work in concert toward enhancing the clinical outcomes of the patients. Nurse-patient collaborations are particularly essential as they enable the creation of a relationship that ensures patients participate in their treatment processes (Dahl & Crawford, 2017). Effective strategies for collaborating with patients and their families include being respectful and maintaining professionalism when communicating with the patients, active listening, acknowledging the patient’s feelings and thoughts, sharing ideas and knowledge on the treatment processes and plan of action, and communicating openly with simple language.

Maintaining professionalism and respect are fundamental in healthcare communication. Professionalism and respect during patient-provider communication allow for the creation of a connection while maintaining professional boundaries. It allows the patients to express themselves freely to the provider. Communication processes should also be open and should use plain language that is easily understandable to the patients. Ideally, this removes any element of ambiguity during the communication process and enhances the patient’s understanding of whatever is being discussed.

Open communication also overcomes barriers to communication that may be attributable to cultural and language barriers between the nurse and the patients. Acknowledgment of the patient’s thoughts and feelings also fosters a better relationship between the healthcare provider and the patient. It also creates a sense of belonging and enables a stronger connection between the provider and their patients. These strategies, upon use, draw patients closer to the providers enabling their participation in the treatment processes. Attainment of an effective collaborative relationship between the nurse and their patient can enhance the efficiency of the care processes. This is because the nurse can share the treatment processes with the patients and influence them into consenting to clinical interventions that they may otherwise not have consented to.

Aspects of Change Management that Directly Affect Elements of Patient Experience

Change management in healthcare offers a framework for transforming various aspects of care intended to improve patient experiences. Change management is essential in the management of mental illnesses and other chronic disorders as it integrates newer approaches or evidence-based practices into the care process to optimize the care process. According to Gonzalez (2022), change management in healthcare is a delicate process that requires an alignment of the organizational objectives to the change process. Therefore, the change management process requires effective healthcare leaders who can inspire the change process to their subjects, thus ensuring a seamless flow of the provisions of the change to the healthcare providers who effect the changes on the patients. Patient experiences such as medication administration, general patient handling, and the design of therapeutic plans can considerably be affected by specific aspects of a change management process.

The change management process in healthcare begins with acknowledging and understanding the need for change. Subsequently, this is followed by communicating the need to other stakeholders within the healthcare system. Healthcare leaders bear the responsibility of communicating the needs to other members of the healthcare teams. The communication should be precise and effective to enable a better understanding of the need. A change plan is then developed, and the change is implemented. Most healthcare changes focus on patient experiences as they inform the overall effectiveness of the care processes (Cleary et al., 2019). Patient experiences are also indicators of quality and, therefore, should be safeguarded at all times. Patient-centered approaches in mental health management have been implicated in optimized clinical outcomes underpinning its necessity.

The Rationale for Coordinated Care Plan

Care coordination is a caring approach that draws multiple healthcare professionals into the care process. Care coordination presents a paradigm shift from the conventional care approaches by exploiting various expertise available in the healthcare system. This approach maintains superiority in chronic care and has been lauded for its effectiveness in managing these patients and in the realization of optimal outcomes. The superiority of the coordinated care approach is due to its ability to tap into the expertise of varied healthcare professionals as well as its ability to tailor their individuals’ values and skills toward the attainment of quality care (Menser, 2017). In this regard, these healthcare professionals work collaboratively and in concert with each other in clinical teams to design treatment plans for the patients and monitor the patients. The benefits of this approach are seen in-hospital care and in the care continuum at home or in care institutions.

The effectiveness of a care coordination plan is dependent on the ability of the individual healthcare providers to work concertedly and understand their roles. This notwithstanding, ethical decision-making remains a gold standard in clinical decision-making processes involving mental healthcare. Ethical decision-making involves making decisions that respect the rights and intrinsic values of the patients. Ethical decision-making in the management of mental illnesses involves the application of evidence-based practices in the treatment process (Tønnessen et al., 2017). Providers must also use the provisions of the ethical codes to inform their decision-making. In this regard, only healthcare strategies that improve the patient’s well-being should be utilized. Additionally, informed consent must be obtained before the commencement of interventions that require informed consent. Patients must also reserve the right of choice regarding their treatment process. Notably, in mental health management, ethical challenges sometimes exist that may present ethical dilemmas to the caregivers. For instance, some patients may not be competent enough to give informed consent due to cognitive decline. In this regard, the caregivers may use their ethical judgment to make the best decision for the patient.

Potential Impact of Specific Healthcare Policies

Several policies have been developed to guide nursing practice and healthcare provision in its entirety. Examples of healthcare policies that impact nursing practice include the Affordable Care Act (ACA), the Health Insurance Portability and Accountability Act (HIPAA), the Mental Health Act (MHA), and Health Information Technology for Economic and Clinical Health (HITECH), among others. The ACA is a provision that ensures increased access to mental healthcare for all Americans who require mental healthcare. The HIPAA act safeguards the privacy of patient health information and requires that they give their consent for it to be shared. In addition, HITECH has enabled the integration of healthcare technologies that enhance the effectiveness and efficiency of care processes. Lastly, the MHA sets standards for mental healthcare management and recommends that information on mentally ill patients remain confidential. These provisions protect the rights of psych patients and ensure that they receive quality care. Snowden (2018) asserts that policies that regulate mental health care have been pivotal in enhancing access to care and have significantly reduced healthcare disparities that were evident in some American minorities.

Raising Awareness of Nurses’ Roles in Care Coordination

Nurses play a vital role in care coordination and the care continuum. Nurses’ role in the healthcare provision stream places them in a strategic position to coordinate care processes. Nurse leaders in this role have the responsibility to participate in the clinical decision-making processes as well as to ensure that clinicians do their duties. They are also responsible for coordinating patient surveillance and health promotional programs on mental health, such as mental health awareness. Nurses must understand that their roles have shifted from traditional handling of patients to more advanced roles of patient education and health advocacy. As nurse practitioners, they must be able to embrace this change and equip themselves with the necessary skills required in policy-making as part of health advocacy and knowledge on preventive approaches to chronic illnesses. Subsequently, this will enable them to impact the patient population in ways that patient handling would not have realized. In addition, the care continuum provides an area in care provision in which specific nursing skills in patient handling and management are necessary. Nursing care remains pivotal in institutionalized or home-based care for mentally ill patients. This is because of the care requirement that these patients often require. Nurses can assist in administering medications and assisting those with functional disabilities.

In conclusion, healthcare collaboration remains an important yet underutilized area in chronic care. Its utility requires that each healthcare professional drawn into the healthcare team understands their role and works in concert with other healthcare providers to ensure the safety of the patients. Effective collaborative strategies have enhanced healthcare relationships and thereby improved the efficiency of care processes. Transformation of care processes often warrants changing processes to the therapeutic care plans of the patients. Change management strategies ensure the integration of these changes into mainstream care, resulting in improved care outcomes. Improved care outcomes can also be assured by employing coordinated care approaches in the treatment process. Nurses play a vital role in care coordination in chronic care. This is informed in part by their set of skills in patient handling, education, and health advocacy.

Introduction

All nurses must be cognizant of the care coordination process and how safety, ethics, policy, physiological, and cultural needs affect care and patient outcomes. As a nurse, care coordination is something that should always be considered. Nurses must be aware of factors that impact care coordination and of a continuum of care that utilizes community resources effectively and is part of an ethical framework that represents the professionalism of nurses. Understanding policy elements helps nurses coordinate care effectively.

This assessment provides an opportunity for you to educate your peers on the care coordination process. The assessment also requires you to address change management issues. You are encouraged to complete the Managing Change activity.

Preparation

Your nurse manager has been observing your effectiveness as a care coordinator and recognizes the importance of educating other staff nurses in care coordination. Consequently, she has asked you to develop a presentation for your colleagues on care coordination basics.
